Clinical Pharmacist II, Ambulatory Care supports the pharmaceutical care vision, mission, and goals of YNHHS by demonstrating proficiency in all ambulatory clinical pharmacist skills. The pharmacist will collaborate with other health care professionals in order to provide comprehensive and coordinated patient care as well as plan and implement strategies for optimizing medication regimens, patient safety, and outcomes.

Responsibilities

  • 1. Drug Therapy Management
    • 1.1 Assumes responsibility for the delivery of pharmaceutical care and medication therapy management, including medication reconciliation, to patients that are deemed high risk or by provider referral
  • 2. Interpretation of Clinical & Pharmacokinetic Lab Data
    • 2.1 Demonstrates knowledge of basic pharmacokinetic principles.
  • 3. Medication-related issue identification and resolution
    • 3.1 Documents Medication Use Variances
  • 4. Drug Information/Education and Scholarly Activities
    • 4.1 Provides drug information to physicians, nurses, and other health care professionals.
  • 5. Communication and Service Requirements
    • 5.1 Understands role as integral member of the health care team.
  • 6. Education/Scholarly Activities
    • 6.1 Performs patient counseling
  • 7. Leadership
    • 7.1 Demonstrates leadership and decision making ability as lead/charge pharmacist
  • 8. Regulatory Compliance
    • 8.1 Ensures environment of care meets regulatory standards as it pertains to medication managment and storage.
  • 9. Continuous Quality/Safety Improvement
    • 9.1 Develops collaborative drug therapy management agreements with providers, enabling pharmacists to act as physician extenders

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree.

EXPERIENCE

Completion of an ASHP accredited PGY1 pharmacy residency with an ambulatory care focus or equivalent experience (2 post-graduate years in an ambulatory care setting) is required. Completion of PGY2 pharmacy residency in Ambulatory Care and Board Certification in Ambulatory Care Pharmacotherapy are preferred.

LICENSURE

Connecticut Pharmacist License or eligible; Board Certification in Ambulatory Care is preferred.

SPECIAL SKILLS

This individual should demonstrate in-depth knowledge of pharmacology and therapeutics, and understand pharmacy systems, ambulatory operations, and formulary management. Excellent communication skills and ability to work with others are required. Excellent communication skills with patients and providers are essential.

PHYSICAL DEMAND

Some travel is required. Valid driver's license and in good standing with DMV of CT.
