Pharmacy Specialist - Ambulatory Clinical (Embedded Clinic) - 40 hrs/wk

GENERAL SUMMARY: The Pharmacy Specialist provides oversight and leadership for appropriate drug use within their region of the health system. They work closely with the medical and administrative leadership as well as the primary care providers to develop and implement strategies which continuously improve the quality of patient care at the best value for the patient and our health system. The Pharmacy Specialist reviews patient drug therapy, interview the patient and/or family, monitors drug therapy responses, and suggest modifications to drug therapy regimens to optimize the quality of care provided by the primary care team. He/she works with utilization management reports, corporate data stores, and information services to optimize drug therapy in their population. He/she will assist in identifying opportunities to streamline patient care, improve the provision of pharmacy services, decrease waste, and rework in the region, and improve drug utilization. The Pharmacy Specialist will be instrumental in guiding the further development of other pharmacist on the health care team.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

For Ambulatory Clinical Pharmacy Specialist:

* Provides comprehensive medication and disease state management activities via a Collaborative Practice Agreement (CPA), as an integral part of the care team. The expanded scope of practice includes, but is not limited to, initiating, modifying, discontinuing medications, ordering vaccines, ordering and evaluating laboratory data, and designing and implementing medication, monitoring, and adherence plans.

* Collect and review patient medical and medication histories, assess vital signs, facilitate ordering of medical equipment, place referral for patient follow-up in specialty clinics/departments, and facilitate prior authorization with appropriate pharmacy partners across the system.

* Will provide disease state and medication education and provide care coordination and transition of care services.

* Using a data-driven approach to decision making, will support population health strategies and participate in value-based initiatives.

* Will serve as a conduit between HFH acute care, outpatient, mail order and specialty pharmacies and the patient's provider. To decrease total cost of care, improve patient safety and satisfaction, and clinical outcomes, will appropriately insource prescriptions and specialty medications internally.

* As part of the High-Value Pharmacy Enterprise, will participate in ongoing research, scholarship, quality initiatives, projects, committees, policy development, and formulary management strategies.

EDUCATION/EXPERIENCE REQUIRED:

For Ambulatory Clinical Pharmacy Specialist:

Completion of a PGY1 Pharmacy Practice Residency required. Experience (3-5 years) in ambulatory care setting preferred. Completion of PGY2 Ambulatory Care/Primary Care Specialty Residency preferred. Board Certification (BCACP or in a relevant area of clinical practice) preferred.

* If practicing in primary care should have experience in various disease states including, but not limited to, diabetes, hypertension, dyslipidemia, asthma, COPD, and congestive heart failure.

If practicing in specialty clinics, should have experience in various disease states within that specialty.

* Registered pharmacist in the State of Michigan.

* Doctor of Pharmacy degree or equivalent experience with evidence of advanced clinical training and knowledge in patient care and applied therapeutics. Postgraduate experience in primary care or managed care is strongly desired.

* Prior experience and knowledge or integrated health systems

* Exemplary oral and written communication skills as evidence with prior examples of effectiveness implementation strategies with a variety of system personnel and patients. Ability to lead work groups effectively.

* Prior experience working with relational data bases.

* Knowledge of Pharmacoeconomics and biostatistical applications in drug therapy decision making.

CERTIFICATIONS/LICENSURES REQUIRED:

Registered pharmacist in the State of Michigan
