Provide comprehensive care for patients with back, neck, hip, and knee pain using a biopsychosocial model, considering the interplay of biological, psychological, and social factors. Collaborate with a multidisciplinary team to address the full spectrum of patient needs, including the impact of social determinants of health. Develop integrated treatment plans that optimize function and quality of life, with a focus on whole-person well-being. This role requires a strong commitment to patient-centered care, excellent communication skills, and a passion for improving the lives of those with chronic pain.

The Position:

Musculoskeletal Integrated Practice Unit - Grand Rapids, MI - ICC Beltline

Full Time Monday through Friday 8:00 am to 5:00 pm

Experience in PM&R, Neuro or Orthopedics Required

Examine myotomes, reflexes, sensation

Review:

  • Mental health factors
  • PROs

  • Social barriers

  • Sleep disorders

  • Lifestyle factors: diet/nutrition, exercise, tobacco use

  • Past interventions trialed and relative outcome

Patient education regarding condition - health and wellness orientation

Shared decision-making regarding treatment options

Opioid weaning support when needed

Referral to appropriate services:

  • Imaging

  • PT: MSK / Spine Specialist

  • Pain Medicine (interventionalist)

  • Pain Psychology

  • BHS / LMSW

  • Lifestyle Medicine / Bariatric Medicine / Health Coaching

  • SUD Specialist

  • Psychiatry

Care Plan, shared decision making, goal setting

Qualifications:

Nurse Practitioner:

  • Required Master's Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master's, post-master's or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
  • 3 years of relevant experience current, relevant clinical experience
  • Previous experience functioning in a collaborative role as a Nurse Practitioner
  • LIC-Nurse Practitioner (NP) - State of Michigan
  • CRT-Basic Life Support (BLS)
  • CRT-Neonatal Resuscitation Program (NRP)
  • CRT-Pediatric Adv Life Support (PALS)

Physician Assistant:

  • Required Master's Degree Graduate of an accredited Physician Assistant educational program
  • LIC-Physician Assistant - State of Michigan
  • CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants
  • CRT-Basic Life Support (BLS)
  • CRT-Pediatric Adv Life Support (PALS)
  • CRT-Neonatal Resuscitation Program (NRP)
