FTE: 0.5 (part-time), Shift: Day, Schedule: TBD We are looking for an empathetic & companionate individual to join our team as a Care Navigator & Data Collection Coordinator! The Care Navigator & Data Collection Coordinator will work with patients who have opioid use disorder (OUD) to help navigate them into community services for medications and support to work towards and maintain recovery. The role will interact with multiple disciplines including the OUD RN, staff RN's, providers - ED/hospitalists, case management, social work. Department Specific Requirements:
Position Summary The Care Navigator & Data Collection Coordinator for the Medication Assisted Treatment (MAT) program is an integral member of a multidisciplinary community network, providing clerical/administrative coordination, data collection to meet Grant requirements and is the point of contact for patients with opioid use disorders in receiving medication assisted treatment, chemical dependency, behavioral health, and support services. This position involves interaction with the patient and professionals and requires professionalism, good judgment, discretion, good public relations, customer service, data collection/collation/reporting & data management skills, clerical and organization skills. This position requires extensive interaction with both internal and external contacts.Requirements

Pay and Benefit Expectations We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $20.47 - $29.45 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
