Position Summary

The Community Health Worker is part of an interdisciplinary care team that assists in care coordination, informing patients on community resources, and improving population health. Responsibilities
  • Performs cross continuum care activities, which may include coordinating care between primary care clinics, specialty care clinics, urgent care clinics, or other sites of care, as appropriate.
  • Engages in outbound communication to patients and keep them engaged in their care, set-up appointments, and prompt them to act.
  • Helps patients access community resources which may include scheduling appointments.
  • Establishes contact with community partners, travel is required.
  • Develop tools to educate peers on patient advocacy and community resources.
  • Participates in community health fairs and events.
  • Educates the care team on available community resources.
  • Participates in quality improvement activities.

Requirements

  • High School Diploma or equivalent
  • 2 years of health care related experience
  • Current Washington State Driver's License; and proof of a safe driving record (which meets established MHS Standards) obtained by a motor vehicle report from the appropriate state.
  • Experience in utilization management, community resources, and/or financial and quality issues affecting patient care, preferred
  • Community Health Worker experience, preferred

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $24.33 - $35.01 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
