HonorHealth is looking for an RN Ventricular Assist Device (VAD) Clinical Manager to support our new, inviting and exciting Program. This is a great opportunity to showcase your knowledge skills and ability while partnering and collaborating with our Providers, Program Leaders and other pertinent staff across the Network to establish the VAD Program. The ideal candidate must meet the minimum requirements of a BSN or MSN along with a minimum of two (2) years of experience in a specialty area of training (ICU, Critical Care preferred) while showing clinical proficiency in the care of this specialized patient population. A current Arizona RN license along with a current ACLS card is required for the role. To ensue success, leadership experience, exposure to quality reports (monitoring outcomes) and process improvement are crucial along with good, confident communication skills, highly initiative, a high attention to detail and the ability to work independently are vital to the role. Creating pathways, algorithms and strong leadership are additional skills that will add value to the team.

Responsibilities

Job Summary
The Ventricular Assist Device (VAD) Clinical Program Manager (CPM) is responsible for the coordination of the HonorHealth VAD program operation, ensuring the delivery of high-quality and cost-effective care, based on competency and evidence-based clinical practice and research. This position will be responsible for overseeing financial management of the VAD program and will work closely with physicians and administrative leaders to provide applicable clinical expertise, clinical leadership, and programmatic development collaboration. The VAD Clinical Program Manager will work closely with the Network Director and Medical Directors to develop strategy.

The CPM will coordinate the clinical evaluation of potential VAD candidates in the inpatient setting and participate in the clinical management of pre- and post VAD implant patients in collaboration with the clinical team and Advanced Heart Failure Specialist. The CPM will follow patients in the outpatient/ambulatory care setting, ensuring continuity of care across the continuum.

This position emphasizes health promotion and maintenance, and the management of patients with acute and chronic Heart Failure. This role works collaboratively with members of the healthcare team to provide education and care appropriate to the potential and identified VAD patients. The VAD CPM will serve as a clinical expert and leadership consultant to support education for patients, care givers, nursing staff and community healthcare professionals on ventricular assist devices, VAD equipment maintenance, and safety measures. The VAD CPM will ensure compliance with organizational guidelines, DNV accreditation requirements, and other regulatory agencies.

The VAD CPM will provide clinical and programmatic oversight of affiliated VAD support staff in collaboration with organizational leaders to foster a standardized and skilled approach to building an evidence-based program. In addition to providing both direct patient care and supervision of VAD support staff, the VAD CPM will participate in an on-call rotation, lead staff evaluations, education, and training. The VAD CPM will participate in recruitment, onboarding, and retention within the practice and work collaboratively with CVSL physicians and administrative leaders to bridge or align practice-based or new programs and services with the service line's hospital-based programs and services.

The VAD CPM adheres to and functions within the scope of practice defined by the Arizona Board of Nursing and the organization. The CPM has knowledge of professional nursing practices to their full scope of practice and can apply knowledge to evidence-based nursing practices and utilizes a shared governance model for decision-making. Nursing practice focuses on performance and quality standards that act in accordance with the organization's initiatives.

This individual possesses an ability to work with all levels of staff within a multidisciplinary group, with a significant level of administrative and clinical autonomy, highly motivated and be able to work independently. Serving as a clinical expert, supervisor, and program leader, and functions as a mentor and educator.
  • Possess the knowledge of how to collect health data in a systematic and ongoing manner, prioritizing data collection as determined by the patient's immediate condition or needs and involving the family. Analyzes the assessment data in determining diagnoses, and utilize the nursing diagnosis to develop, implement, evaluate, and revise an appropriate patient plan of care that is family centered, developmentally and age-appropriate, and culturally relevant.
    Identifies expected outcomes individualized to the patient and their specific health problems. Develops plans of care that are individualized to the patient's condition or needs, reflecting current nursing practice that is evidence-based, providing for continuity of care, and are family centered.

    Implements interventions in the plan of care. Initiates treatments, medications, emergency, and resuscitative measures based on appropriate utilization of standing orders, policies and procedures. Provides nursing care to meet the physical, emotional, spiritual, and social-cultural needs of the patient and family utilizing a family-centered approach to care delivery.

    Evaluates the patient's progress toward attainment of outcomes. Documents observations, nursing interventions, therapeutic measures, multi-systems monitoring data, and other data relevant to the patient's care in a retrievable form. Provides ongoing evaluation of the quality and effectiveness of the care plan to ensure that they maintain and enhance patient outcomes that promote the delivery of cost-effective high-quality healthcare.
  • Evidence-Based Program/Services Development:

    The VAD CPM will function independently and collaboratively to direct the planning, development, and implementation of practice-based VAD program and services. The CPM will work closely with service line physicians and administrative leaders to lead, advocate and support program innovation.

    The VAD CPM Leads, advocates, and/or supports the development of key systems to provide the best patient experience and access points with efficient VAD patient care navigation. Maintain environmental safety and orderly patient flow. Coordinates activities and interacts effectively with other departments and medical staff to manage the patient care environment. Will organize, lead, and collaborate in the development, implementation, and maintenance of standardized, multi-disciplinary care processes, protocols, and tools based on current evidence-based practice and/or established clinical guidelines.

    The VAD CPM will identify gaps in care continuum nursing/provider knowledge and works collaboratively with nurse educators/navigators/coordinators to deliver education opportunities that safeguard the use of best clinical practice and up-to-date guidelines and care standards. Collaboratively develops goals and expectations related to the program, clinical care, and quality performance.
  • Education and Oversite:

    The VAD CPM will provide education and support to community health care professionals who may encounter VAD patients. Acts as a consultant to referring community health care specialists and primary care providers. Provides VAD program information during the negotiation of managed care VAD contracts. Assures that all VAD patients have an identified PCP and that these providers receive correspondence and educational material on their VAD patients. Develops resource materials for referring physician and strategies to enhance communication between the VAD program and outside providers.

    The VAD CPM identifies competency/learning needs and collaborates with department directors, clinical coordinators, and staff in designing orientation and ongoing educational activities related to the care of VAD patients. Develops educational materials and provides classes for nursing staff and VAD team members.

    The VAD CPM will continue to monitor new/updated expert consensus recommendations to ensure that assigned specialty practice is delivering precision care.
  • Reporting/Data Management:
    The CAD CPM supports CVSL Pillar Chiefs, Medical Directors, and Quality Consultants in quality and registry process improvement including review, planning, and initiative selection required to drive better clinical outcomes.

    Documents appropriate information in the patient's medical record. Responsible for collaborating with the quality team and ensuring adherence to INTERMACS database requirements. Prepares statistical reports and analyses data and reports on progress, adverse trends, and appropriate conclusion and recommendations. Maintains a patient management system and collects and documents required regulatory and program-specific data.
  • Professional Development:
    Participates in professional organizations and attends continuing education activities to maintain knowledge of current trends and practices. Evaluates one's own nursing practice in relation to professional practice standards and relevant statutes and regulations of the system and department policies and maintains current knowledge in nursing practice. Systematically evaluates the quality and effectiveness of nursing practice.

Qualifications

Education
Bachelor's Degree or Nursing OR Master of Science in Nursing - Registered Nurse / Clinical Nurse Specialist or Advanced Nurse Practitioner Required

Experience
2 years Clinical Experience in the assigned specialty area (ICU, Stepdown, VAD etc,)
Must demonstrate clinical proficiency in the care of the specialized patient population Required
