The Center will feature a variety of co-located partnerships to allow us to bring community-centered programs to expand the capacity and reach of our mental health and addiction services. Key Position Details: Job Description: Utilizes critical thinking skills in the counseling process of assessment, planning, implementation, and evaluation of patient care. Provision of patient care is based on care management, quality/resource, stewardship, patient advocacy/customer service and role modeling. Accountable for professional practice of counseling and participation in the interdisciplinary care delivery model. Principle Responsibilities Assessment/reassessment Perform assessments related to substance use, as needed. Collaborate with team to reassess patient and progress in treatment. Crisis support and management. Treatment Planning Complete treatment plan per department process. Documentation of patient, and family if appropriate, participation in development of treatment plan. Review treatment plan with patient at regular intervals. Initiate and support referrals to internal/external providers and agencies. Coordinate discharge and recovery and transition planning. Provision of Treatment Facilitate group and individual counseling, as identified by treatment plan. Coordinate treatment with other community agencies/providers, when appropriate. Provide patient and family education, as needed. Other duties as assigned. Required Qualifications Associates degree in Behavioral Sciences Preferred Qualifications Bachelor's degree in Behavioral Sciences Experience in a treatment program Licenses/Certifications Licensed Alcohol and Drug Counselor - MN Board of Behavioral Health & Therapy required upon hire Must meet at least ONE of the requirements below: Current BLS certification from the American Heart Association Current BLS certification from the American Red Cross Allina in-house BLS training (within 30 days of hire Physical Demands Sedentary: Lifting weightUp to 10 lbs. occasionally, negligible weight frequently
