Responsibilities:

  • Implement programming for residents on a consistent basis.

  • Facilitate implementation of the treatment plan.

  • Facilitation of groups, completion of daily paperwork including daily logs, group logs, session notes.

  • Crisis intervention

  • Collaborate with providers

  • Monitor residents' medication, refills, document medications and changes, provide safe storage and also assist while attending appointments.

Qualifications:

  • Grade C02: Case Manager I: HSD/HiSet/GED or Associates degree required,

  • Grade C04: Case Manager II: Requires a Bachelor's degree; preferably in the field of psychology or human services

  • 1+ years experience in social services, the substance abuse fieldand/or working with program's population preferred

  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes, including transporting clients in your own vehicle or a company vehicle
