MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Current unencumbered licensure with the WV Board of Practical Nursing, or appropriate state board where services will be provided, as a practical nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).2. Obtain certification in Basic Life Support within 30 days of hire date.3. West Virginia state criminal background check, and federal, if applicable, required for DHHR regulated areaEXPERIENCE:1. Five (5) years of LPN experience.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Assigns duties and coordinates clinical services in absence of clinical manager. Applicable to multiple clinic locations including offsite locations.2. Coordinates ambulatory nursing care. Ensure staffing, delegates assignments, update nursing staff on new policies/procedure and ensure implementation, monitor effectiveness for clinical support staff at facility. Develops and distributes daily work assignments and monthly schedules.2. Keeps informed of special physician orders concerning patients.3. Investigates and resolves complaints or refers unusual problems to clinic manager.4. Responds to patient teaching opportunities to enhance patient compliance and safety.5. Identify family involvement in patient compliance.6. Performs all LPN direct nursing care duties within licensed scope of practice, assess patient concerns, including type and level of care needed through triage.7. Participates in problem solving and decision making. Acts to ensure staffing and patient needs are met.8. Obtain medical information from patients. Take height, weight, blood pressure, and vital signs of the patient. Screen patient for chief complaint and pertinent health data. Record data gathered in patient's medical record and communicate data gathered to physician.9. Works closely with office assistant and physician to facilitate patient's clinic visit. Escorts patients to and from exam room. Assists physician with examination if needed. Provides emotional support to patients and/or families. Assists with scheduling when necessary.10. Responsible for coordination of medical supply orders for clinic. Exercises proper stewardship through the appropriate use of supplies, equipment, and time.11. Provides on-going feedback to clinical support staff concerning job performance. Coaches and counsels employees under direction of manager.12. Serves as a mentor for all clinical staff to help foster staff development.12. Assists in training LPN staff.

1. Heavy/Hard work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, positioning of patients, standing, walking; carrying of patients, materials and equipment weighing 40+lbs. This is performed on a daily basis.

1. Standard clinical environment. Volume of patients is very high. Position is frequently exposed to bloodborne pathogens, bodily fluids, toxins, and chemicals.


SKILLS AND ABILITIES:

1. Must have the ability to work successfully under highly stressful conditions, and must be capable of adapting to varying workloads and work assignments on a constant basis.2. Excellent written and oral communication skills.3. Must be skilled with computer systems used by both physician offices and hospitals to capture data accurately and consistently.4. Ability to function as a team member in respect to organization, communication, and task prioritization. Must also be able to work weekends, holidays and all shifts.
