004142 The Heart Transplant Coordinator (HTC) is responsible for the coordination of required testing, management and follow up across the continuum from evaluation, pre-transplant (waitlist management), during transplant and post-transplant. The HTC will maintain regulatory integrity and adherence to established regulations throughout all processes. Educate patients, families and other care givers on the transplant process through the continuum. The HTC will have on-call responsibilities for the heart transplant program, which includes collaborating with Gift of Life, medical and surgical teams on the inpatient and outpatient services, all members of the multidisciplinary team and consulting specialists as indicated. The transplant coordinator will also support the outpatient transplant clinics. The position includes, but is not limited to the following essential functions. ESSENTIAL FUNCTIONS:
EDUCATIONAL/TRAINING REQUIREMENTS: Bachelors of Science in Nursing Required Previous critical care and/or CICU/ CVICU/CTICU experience preferred CERTIFICATES, LICENSES, AND REGISTRATION:Registered Nurse CCTC certification within 2 years of becoming eligible to sit for the exam EXPERIENCE REQUIREMENTS:Minimum of 5 years nursing experience with at least two (2) years in critical care, transplant, disease specific OR minimum of 3 years clinical nursing experience in transplant, critical care, disease specific.
