Position Title:Direct Patient Care Pharmacist- University of Oklahoma Medical Center Department:Pharmacy Job Description:

General Description: Under the general direction of the Associate Vice President of Pharmacy, the Direct Patient Care Pharmacy Specialist is responsible for the support of hospital staff by providing direct patient care to hospitalized patients through collaborative management of the medication- use process (ordering, dispensing, administering, monitoring, and educating).

Essential Responsibilities:

  • Collaborate with the patient care team to provide care to patients in a safe and effective manner.
  • Review medication orders and gather medications from pharmacy or automated dispensing cabinet for administration including management of intravenous infusion pumps.
  • Administer medications and immunizations as prescribed.
  • Evaluate laboratory results necessary for monitoring outcomes of medication therapy.
  • Interpret data related to medication safety and effectiveness.
  • Document the care provided in patients' medical record.
  • Perform a limited patient assessment including point of care testing, vital signs and other relative information as needed for patient care.
  • Troubleshoot medication related issues including but not limited to automated dispensing machine and infusion pump management.
  • Assist in administration of high alert medications in effort to reduce medication errors.
  • Perform medication reconciliation and accurately document in the electronic medical record.
  • Interpret medication orders and obtain necessary information for clarification from prescribers if necessary.
  • Evaluate medication orders for appropriateness and verify orders prior to dispensing.

Minimum Qualifications:
Education: Graduate of an accredited pharmacy degree program. Completion of PGY-1 Pharmacy Residency preferred.
Experience: One (1) year experience in a hospital or direct patient care setting preferred.
Licensure/Certifications/Registrations Required: Licensure as a Doctor of Pharmacy by the Oklahoma State Board of Pharmacy. Certificate as a preceptor by the Oklahoma State Board of Pharmacy when eligible.
Knowledge, Skills and Abilities: Knowledge of hospital practice and patient care.
