Position Details As a member of the Intensive Transition Services (ITS) Team, the Licensed Transition/Care Coordinator is qualified in the human services field and possesses the skills and experience necessary to provide clinical therapeutic interventions, care coordination and supportive services to program individuals with a mental health diagnosis and Intellectual/Developmental Disability (Dually Diagnosed). Services are provided in a multi-disciplinary team setting and done in under the clinical supervision of the Program Director. The ideal candidate will possess the following qualifications: * Master's degrees in social work, Psychology, or another human service related field * Current and unrestricted California license with the Board of Behavioral Sciences such as Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT)or Licensed Professional Clinical Counselor (LPCC) * Three years experience in the behavioral health field working with individuals with developmental disabilities Preferred: * At least one year of experience of case coordination and case management services Equal Opportunity Employer Minorities/Women/Protected Veterans/Disabled | |
