SUMMARY

The Internal Medicine Clinical Pharmacist is a licensed pharmacist who is responsible for applying knowledge, skill, competence and clinical expertise in our internal medicine intermediate care unit. The clinical pharmacist will work on an internal medicine unit as a patient care provider in addition to facilitating quality initiatives for monitoring of inpatient therapy such as renal function adjustment, anticoagulation and drug interaction monitoring. The roles of this clinical pharmacist are that of advanced practitioner, educator, consultant, leader and researcher.

The Clinical Pharmacist applies his/her knowledge of, and clinical training in, pharmacotherapy to directly influence and impact physician prescribing to improve the use of medications throughout the Health System and to enhance positive drug therapy outcomes. The Clinical Pharmacist, acting as an integral member of the healthcare team, assumes responsibility for the appropriate selection and monitoring of medications through direct patient care activities as well as serving as a liaison between the pharmacy and other healthcare practitioners.

Other functions include precepting pharmacy PGY-1 and PGY-2 residents and doctoral-level pharmacy students, participating in staff development, creating opportunities for pharmacists to practice more clinically and promote rational and cost-effective use of medications at Grady Health System.



QUALIFICATIONS



  • Doctorate of Pharmacy degree plus 1 year post-Phar.D. experience
  • ASHP accredited Residency in pharmacy practice or Doctor of Pharmacy degree at least 5 years of post-Phar.D. experience in job-related pharmacy practice, or Bachelor of Science degree in Pharmacy at least 6 years experience in job-related pharmacy practice at GHS
  • Fully licensed as a Pharmacist in the State of Georgia or, eligible for full licensure within 120 days through reciprocity or exam
