SUMMARY: We are currently seeking a Social Worker I to join our team. This full time role will work days. This position will be working to link newly diagnosed and out of care individuals to care. This is a grant funded role, currently funded through August 2025 with potential for extension. Purpose of the position: Under general supervision, and in collaboration with the health care team, this position evaluates individualized social treatment plans requiring the application of social work theory and methods in the prevention, treatment, or resolution of psychosocial problems RESPONSIBILITIES:
