Description

Case Manager II

We are seeking a Case Manager II to work on our Waiver Case Management team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Waiver Case Management involves more behind-the-scenes work supporting client needs than time meeting with clients. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules. This is a full-time hybrid position. We value a work-life balance and our case loads are on average 30 within a year.

Hours: Full-Time, Monday- Friday, business hours 8am-4pm

Locations: Twin Cities counties, Minnesota | Hybrid **Physical presence is required**

Hiring Ranges: $23.00 - $26.00/ Hourly, based on qualifications

Hiring Bonus: $1,000 to external candidates

Main Job Duties:

  • Must be able to have on-site presence in the Twin Cities, Minnesota
  • Attending bi-annual meetings with clients to monitor services
  • Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
  • Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
  • Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed
  • Making referrals and authorizing services based on needs
  • Maintaining records and documenting all coordination activities
  • Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards
Requirements

Requirements for All Case Manager positions:

  • Passing all background checks
  • Valid driver's license, excellent driving record and current auto insurance

Education and Experience Required for Hennepin, Anoka Counties positions:

  • Bachelor's Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field;

OR

  • Bachelor's Degree from an accredited four-year college with a major in any field, AND
  • One year of experience as a social worker/case manager/care coordinator in a public or private social service agency

Education and Experience Required for Ramsey County positions:

  • Bachelor's Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND
  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions

Preferred Experience for Case Manager II all counties:

  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions
