Job Summary
The Quality Navigator is responsible for coordinating activities involving patient populations through risk stratification activities requiring one on one support in coordinating care activities, supporting and assisting patients to navigate through the steps in obtaining their necessary and appropriate care. The Navigator works collaboratively with the primary care providers, staff, and care managers in support of their efforts to address patient care needs and opportunities. To effectively advocate for patient needs, the Navigator may contact patients and perform/gather routine health screening data. The Navigator works with patients and clinicians to ensure ongoing review of routine and ongoing care opportunities in an effort to meet established quality goals and metrics.
What you will do
* Develop and spearhead initiatives aimed at addressing population health gaps, with a focus on identifying and mitigating health disparities.
* Drive quality improvement projects related to population health, ensuring alignment with established goals and quality metrics.
* Provide guidance to providers and clinical staff on health maintenance and preventive health recommendations. Recommend and implement changes to protocols and workflows to standardize practices.
* Assist patients in understanding available resources, including educating them about appropriate screenings and care opportunities through in-person education, mass communication campaigns, and community events.
* Keep abreast of national clinical standards and evidence-based practices. Collaborate with providers, staff, and departments to develop standardized protocols and workflows based on national guidelines (e.g., USPSTF, ADA).
* Collaborate with EPIC clinical informaticists to strategize and implement workflows, best practice alerts, and health maintenance reminders.
* Attend health fairs and other community events to educate the public on preventive and clinical care recommendations and available resources.
* Serve as a resource for Medicare Advantage and Commercial insurance plans, providing expertise in population health.
Experience Qualifications
* 3-5 years work experience in a clinic setting within a health care organization. Required
Skills and Abilities
* Ability to operate a computer and to understand the language used in working with a specific system or systems.
* Knowledge and understanding of how Electronic Medical Records (EMRs) and Clinical Applications are used in Clinical practice setting
* Excellent interpersonal skills and behaviors
* Ability to form relationships with peers
* Investigates, summarizes, and effectively presents current issues and potential improvement solutions
* Skilled in analyzing, assessing, and creating recommendations for improvement in population health initiatives
* Creating high-level staff education and presentations
* Skilled at communicating information and updates to different levels within the organization
* Ability to prioritize work and meet changing demands
Licenses and Certifications
* Licensed in the State of Wisconsin as a Registered or Practical Nurse OR a Certified or Registered Medical Assistant Upon Hire Required
