Description

This position is responsible for the transportation of patients to and from appointments at the SIU Center for Family Medicine.


Examples of Duties

TRANSPORTATION/VEHICHLE MAINTENANCE 90%
1. Provide assistance/ transportation to patients using SIU Center for Family Medicine's van.
2. Prepares daily transportation schedule based on patient needs.
3. Generates trip sheets for each trip and prepares monthly reports on total trips, mileage and vehicle maintenance.
4. Ensures vehicle gets regularly scheduled maintenance.

FOOD DISTRIBUTION 10%

1. Pick up of orders done at local businesses and food pantries.
2. Deliver items and food bags to patients as needed.
3. Stock food pantry and record usage/maintain montly pantry walk throughs
4. Record distribution activity daily.
5. Ensure the distribution area is stocked with grocery bags, surveys and recipes


Qualifications

  1. High school diploma or equivalent.
  2. Valid/current driver's license as required by the State of Illinois and a CDL class B license or higher, in accordance with the size of the vehicle to be operated by the incumbent.
  3. Six (6) months of work experience as a route or commercial delivery driver or closely related experience to the given position.

NOTE: All necessary CDL endorsements are based on the vehicle to be operated and when required to operate vehicles capable of transporting 16 or more persons as required by the Illinois Secretary of State.
