  • Evaluates and Re-evaluates adult clients with developmental disabilities with the correct selection of evaluation tools, accurate interpretation and reporting of results and the utilization of assessment results to creating a plan with input from clients, family and/or other stakeholders.
  • Select appropriate procedures and/or equipment and applies in a safe, effective manner
  • Trains other staff members in use of procedure and/or equipment as needed
  • Assesses effectiveness of current procedures and/or equipment and modifies or discontinues plan as needed.
  • Demonstrates effective written communication through the completion of all billing and written clinical documentation in a timely and accurate manner.
  • Must be able to communicate internally with clients and coworkers as well as externally with guardians, community case managers, caregivers, transportation drivers, among others.
  • All other duties as assigned





SKILLS & QUALIFICATIONS




  • Bachelor's Degree in Physical therapy from an accredited Physical Therapy school.
  • Certified and Licensed in the State of Wisconsin.
  • Possess valid Wisconsin driver's license, reliable transportation and car insurance
  • Previous related experience in a healthcare setting is preferred.
  • Able to work independently on a broad variety of projects.
  • Able to exercise effective judgment, sensitivity, and creativity to changing needs and situations.
  • Meet internal and external deadlines and produce a consistently high-quality product.
  • The ability to contribute in a team environment and/or independently, to provide excellent customer service.
  • Ability to organize, plan and execute tasks in an efficient and profession manner.
  • Strong written and verbal communication skills.
  • Strong computer skills, especially with the Microsoft suite: (Word, Excel, Outlook).
  • Must have a valid Wisconsin Driver's License and clean driving record.






LICENSES & CERTIFICATIONS




  • Valid Wisconsin Driver's License
  • CPR/First Aid certified within first six months of hire
  • Wisconsin Physical Therapy License
