JOB SUMMARY

The Medication Access Coordinator (Pharmacy Navigator) reports to their respective manager in close collaboration with the clinical ambulatory pharmacy team and serves as the primary liaison for clinical stakeholders regarding pharmacy-based prior authorization programs, refill renewals, prescription assistance programs, and assists the clinical pharmacists in their daily tasks at their discretion. The Pharmacy Navigator is responsible for integrating work, coordinating initiatives, and communicating as necessary across multiple Wentworth Douglass Clinic sites, medication-related prior authorization and support programs, as well as facilitating communication and documentation for clinical stakeholders. The Pharmacy Navigator competes assignments with a high degree of accuracy and precision and documentation is clear, concise, well organized, and timely. The Pharmacy Navigator is also responsible for overseeing workload and balancing quality improvement project management.

ESSENTIAL FUNCTIONS



  1. Support designated clinic sites by assisting with patient medication management and access.



  1. Complete medication prior authorization paperwork for signoff by the clinical care team. Monitor progress of prior authorization status throughout review process and updating appropriate parties as necessary
  2. Assist the clinical pharmacists in their day-to-day workflow in assessing medication adherence, call pharmacies to confirm fills and utilize coupons to ensure we are strengthening patient access, engaging in patient follow-up calls to confirm implementation of regimens as directed by the clinical pharmacist, and other tasks as delegated from the clinical pharmacists
  3. Receive, review, and facilitate requests for medication renewal. Review the requests according to guidelines and protocols. Pend the requests in the electronic medical record (Epic) and route to prescriber for review and signature
  4. Educate/ in-service MDs, RNs, and NPs on reimbursement change


2. Act as a patient resource as it relates to medication access.



  1. Act as a resource for patients on procedures of obtaining prescriptions, generic medications, prior approval requirements, insurance procedures, and other medication related issues
  2. Answer the phone, provide information, and direct calls to the appropriate areas
  3. Build and manage relationships with clinic managers, medical providers, care coordinators, and financial counselors to best serve patients
  4. Assist in resolving pharmaceutical company, clinic, and patient issues


3. Project management and service development



  1. Manage database and track all information for reporting and analytics; maintain reports, spreadsheets, log sheets and forms as necessary.
  2. Train and supervise staff members as needed
  3. Other activities as assigned by the clinical pharmacy team

Qualifications

  • Experience Minimum Required

    • One-year experience working with prior authorization in a health care setting
    • Must have ability to effectively communicate clearly both verbally and in writing


  • Experience Preferred/Desired

    • Prior pharmacy experience desirable
    • 3 years working in prior authorization or pharmacy revenue cycle in a health care setting


  • Education Minimum Required

    • HS Diploma


  • Education Preferred/Desired

    • Pharmacy Technician program
    • Bachelor's degree in business or related field


  • Training Minimum Required

    • Proficient with Microsoft Office, including Excel and Word


  • Training Preferred/Desired

    • Successful completion of an accredited or Board-certified Pharmacy Technician or Medical Assistant training program, or equivalent institutional/retail pharmacy or medical office experience is highly desirable.


  • Special Skills Minimum Required

    • Requires knowledge of laws and regulations related to clinical practice and patient confidentiality.
    • Requires knowledge of medication abbreviation and symbols, medication indication and dosage, and common medication questions.
    • Requires knowledge of medical terminology, abbreviations, and computer technology.
    • Strong written and verbal communication skills in English required.


  • Special Skills Preferred/Desired

    • Experience with electronic medical record system(s) and Outlook is preferred. Experience with Epic is desirable.


  • Licensure and/or Certifications Required

    • Register with the New Hampshire Board of Pharmacy within 15 days of start date of employment as a pharmacy technician


  • Licensure and/or Certifications Preferred/Desired

    • National Pharmacy Technician Certification
