Position Summary:

Responsible for providing short-term intensive case management to eligible households whose tenancy problems are related to a disability.

Essential Responsibilities:

  • Assess needs of individuals/families who are at risk of losing their housing due to disability related issues.

  • Develop service plans to address issues effecting tenancy and complete referrals for on-

  • Complete program evaluation documentation, monthly reports, reports for housing court and client specific case records and in quality improvement activities.

  • Participate in supervision, meetings and trainings.

  • Works closely with tenants, service providers, landlords, and housing court personnel.

  • Ensure timely completion of all reports and paperwork.

  • Able to work independently and keep supervisor informed of service specific issues.

  • Familiar with skills necessary for maintaining independent living and understands how to access resources to support independent living.

  • Participate in other agency committees/work groups as needed.

  • Maintains knowledge base of services to assist in prevention of homelessness

  • Performs other related duties.

Standard Staffing Level Responsibilities:

Complies with established division and program policies, procedures, and objectives.

Attends variety of meetings, conferences, and trainings as required or directed.

Demonstrates use of quality improvement in daily operations.

Complies with all health and safety regulations and requirements.

Respects diverse views and approaches and contributes to maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.

Maintains regular, reliable, and predictable attendance.

POSITION QUALIFICATIONS

  • Grade C02: Case Manager I- HSD/HiSet/GED or Associates degree required

  • Grade C04: Case Manager II-Bachelors degree required; degree in psychology, social work, human services or related field preferred

  • Ability to effectively communicate and develop positive professional relationships with clients, staff and co-workers and outside stakeholders.

  • Strong clinical background and ability to translate clinical theory to real life application.

  • Able to work independently & Complete all work in an accurate, thorough and timely manner.

  • Have basic computer skills.

  • Strong verbal and written communication skills.

  • Functions well in formal work environment of housing court, local housing authorities and legal services

  • Current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle

  • Desire to serve others required

  • Demonstrated strong work ethic, ability to work independently, and ability to contribute to team objectives

  • Must be able to pass a CORI background check
