Job Summary The RN-Home Health Program Coordinator is responsible for coordinating and providing specialized care in one of several home health program areas for patients and clinical staff. The home health programs include wound care, pediatric care, and palliative care. Responsible for coordinating with other disciplines and agencies involved with patient's care.Bonus

Accountabilities *Team Responsibilities: Promotes, maintains, and documents communication between physicians and clinical staff regarding patient care for a home health clinical program. Current home health programs include wound care, pediatrics, and palliative care. Confers with supervisors/managers and attends team meetings. *Documentation Responsibilities: Documents coordination with clinical staff and physicians related to patient care provided by a home health program. Documents direct and indirect nursing care provided by the program coordinator. Maintains an inventory and status of patients served by the particular home health program. *Professional Responsibilities: Provides education and field support to clinical staff relevant to a specific program area. Participates in continuing education and other relevant sources to maintain current nursing expertise in the home health program specialty area. Maintains the highest level of professionalism and conduct at all times. Follows agency policies and procedures. *Clinical Responsibilities: Provides direct and indirect nursing care as required and appropriate to patients needing specialized support from a home health program. Provides direct nursing care for other patients as required by the agency. *Administrative Responsibilities: Tracks and reports out all relevant data relevant to a specific home health program. Participates in chart audits and other related Quality Improvement projects that are relevant to a specific home health program. Submits all paperwork/documentation according to agency policy. *Accepts assignments outside normal duties when such fulfils an agency need. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. Minimum Hiring Requirements Associate Degree in Nursing (AD N) required. BSN preferred. One year experience as a registered nurse. Required Licensure/Certifications Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working. Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance. Other Knowledge, Skills or Abilities Driving is required for this position using the employee's personally owned vehicle. Employee must have reliable transportation and will be expected to adhere to the Prisma Health Driver Safety Policy and specific department driving policies.
