The recruit will assist the Endocrinology physicians in their faculty practice and future programs. NP will provide direct care to adult patients with Type 1 and type 2 diabetes, including medical management and patient education. May teach in a patient education program for Diabetes. May include evening hours, up to two evenings per week, to teach the group classes

Associate may be required to work at multiple locations, including clinics and hospital floor; at both Moses and Weiler campus. Hours are Monday-Friday 830a-5p, except when evening hours are required for educational workshops up to two evening per week.

RESPONSIBILITIES:



  • Nurse practitioner to work in out-patient and inpatient setting for multiple locations throughout Montefiore. NP will provide direct care to adult patients with type 1 and type 2 diabetes, including medical management and patient education.
  • Will manage use of insulin pumps, continuous glucose sensors and related diabetes technology. NP works as part of a team that includes CDEs (RN, NP, RD) and endocrinologists.
  • May also include teaching in an ADA recognized patient education program.
  • In collaboration with physicians and otherhealth care providers, coordinates care ofpatients and evaluates plan of care and patient
  • toward health goals and modifies plan of careaccordingly.
  • Communicate effectively with all members of care team - including patient, referring and consultant MDs, support staff, etc.
  • Prescribes medications and educates thepatient and/or involved others about the drugregimen, interactions and side effects.
  • Orders, performs and interprets diagnosticprocedures and tests with the scope ofprivileging.
  • Initiate referral to appropriate physician after consultation from admitting physicianor to other health care professional of problems that exceed the NP's scope of practice.
  • Provide education to patients and their significant others, doctors, nurses, and other health careprofessionals to improve clinical care.
  • Select, use, and/or evaluate technology, products, and devices appropriate to the specialty area ofpractice.
  • Precept students and mentor other nursing staff.
  • Demonstrates critical thinking skills toproblem-solve and initiate clinical decisionmaking.
  • Monitor and manage acute and chronic illnesses of population served. Order diagnostic and therapeutic modalities, e.g., laboratory, x-ray, ekg, etc. Perform acts of diagnosis and treatment as determined by established agreement between NP's scope of knowledge and training and the collaborating physician's/designee's scope of clinical privileges.
  • Prepare patients for surgery, including patient/family education, informed consent (for any procedures that arebeing personally performed by the NP) and postoperative care.
  • Medical Record Charting Responsibilities:
  • Clearly, legibly, completely, and in timely fashion, describe each service she/he provides to a patient in the hospital and relevant observations. Standard rules regarding authentication of, necessary content of, and required time frames for preparing and completing the medical record and portions thereof are applicable to all entries made.
  • E&M Billing; support work; returning patient telephone inquiries.



REQUIREMENTS:



  • Master's Degree preferred, Bachelor's Degree required in Nursing with specialty NP certification
  • Minimum 3 years experience as an NP
  • Current NY State RN License as well as current NP specialty certification and registration.
  • Ability to manage use of insulin pumps, continuous glucose sensors and related diabetes technology.
  • May also include teaching in an ADA recognized patient education program.
  • Minimum 3 years diabetes experience required.
  • Bilingual (Spanish/English) a plus.
  • Hours are Monday-Friday 830a-5p, except when evening hours are required for educational workshops up to two evening per week.
