Intake Specialist/ Case Manager (Full Time)
Pay range : $24.83 - $30.23

Our Community Support Program (CSP) needs a compassionate individual to fill the position of an Intake Specialist. This role is responsible for the lead in the development of comprehensive health service plans, the daily organizational and treatment /service planning and monitoring client responses to health coordination and prevention activities.

CSP provides case management services to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community. You will assist in orienting new clients and their families to CSP and health home services.

An ideal candidate will:



  • Demonstrate competency in clinical/technical or administrative skills.
  • Work as a member of a multi-disciplinary team, and performs in a fast-paced environment.
  • Transport clients and performs the home visits.
  • Be a self-starter, and tends to multiple tasks simultaneously, and works with minimal supervision.


Education/Licensing Requirements:


  • Bachelors degree preferred in psychology, social work, or other human services related field.
  • AA degree or equivalent education level in the human service field required.
  • One-year experience in community mental health preferred.
  • Completion of the CCSP training.
  • Must have a reliable vehicle, auto insurance and valid drivers license.
