The Patient Care Technician provides direct, age-specific care to a patient or group of patients based on medical orders and the plan of care under the direction of the RN or licensed independent practitioner. Essential Job functions:
Patient Population: Demonstrates the knowledge and skills necessary to provide equitable care appropriate to the age of the patients served on their assigned unit. Demonstrates knowledge and related competencies of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to their age, specific needs and to provide the equitable care needed as described in departmental policies and procedures. Neonates (0-4 weeks), Infant (1-12 months), Pediatrics (1-12 years), Adolescents (13-17 years), Adolescents (13-17 years), Adults (18-64 years), Geriatrics (65 years and older) Employment Qualifications: High School Degree or equivalent Preferred Education: Licensure/Certification Required: AHA BLS HCP Certification or equivalent Licensure/Certification Preferred: N/A Minimum Qualifications: Certification/Completion of one of the following:

Our Hospital at Home program is one of our most innovative and most rapidly growing programs within the CIC that leverages both clinical and technological expertise to provide the ultimate holistic approach to inpatient care. Combining both acute care and home care, the Hospital at Home Patient Care Tech (PCT) role is a unique, exciting, and truly holistic experience that provides direct inpatient level of care in the comfort of the patient's own home. Hospital at Home programs are gaining momentum nationwide, because of the benefits for patients and health systems, including increased patient satisfaction, improved patient outcomes, better performance on value-based metrics and relieving facility capacity constraints. The model operates with a patient-centric focus and a comprehensive, team-based, outcomes-motivated approach to care using population health principles to keep patients healthy. The PCT supports our nursing and medic visiting team during Hospital at Home visits through a paired visit model. The PCT is paired with a nurse or medic during their shift to complete Hospital at Home visits. These visits include management of the patient according to the medical and nursing care plan. Tasks that the patient care tech may complete include but are not limited to: Checking vital signs, reinforcing education on how to use the in-home technology for Hospital at Home, helping patients with ADLs, phlebotomy, and 12 lead EKG. The PCT also supports the virtual command center when not out in the field doing home visits with such tasks as screening phone calls, stock/inventory of medical supplies, equipment, and food/nutritional items, and completing outreach to potential remote patient monitoring patients. Our transitional remote patient monitoring program provides nursing, case management, and provider support for patients graduating from the Hospital at Home program or discharging from a VCUHS hospital.
