About the Kidney Transplant APP Role and Transplant Dept The Advanced Practice Provider (APP) (Nurse Practitioner/Physician Assistant) reports organizationally to the Abdominal Transplant Program Administrative, Nursing, and Clinical Directors. The APP is responsible for providing comprehensive care, assessing, diagnosing, and treating patients in a variety of settings, in collaboration with physicians and other healthcare professionals, including pharmacists, transplant nurse coordinators, social workers, dieticians, financial coordinators, medical assistants, research and administrative staff. The APP will also be responsible for providing education to patients and their families. The APP acts as the central point of contact and maintains open lines of communication between to the surgeons, physicians, and other members of the team. The APP also interacts with external stakeholders such as organ procurement organizations, transplant centers, insurance companies, pharmaceutical representatives, and community providers. The APP may supervise, or mentor other medical staff or students involved in the transplant program. The APP functions in direct collaboration with the nephrologists and surgeons assigned to the Transplant Center. They initiate, facilitate, and develop interdisciplinary collaboration and participate in all aspects of the post-kidney transplant patient's care, providing and maintaining comprehensive documentation in the patient medical record. In collaboration with patients and families, she/he designs a plan of care based on an assessment of physical, emotional, and social needs with an understanding of the pre- and post-transplant protocols. The APP is also involved in some aspects of pre-transplant care and participates on committees and task forces that relate to organ transplantation. The APP also participates in our MDC (multidisciplinary clinics) for pre kidney transplant evaluation, providing a comprehensive history and physical on potential kidney transplant recipients, provide education about the transplant process, and participate in selection committee meetings.
