Summary
Coordinates efforts related to clinical pharmacy initiatives. Provides support to the Therapeutics Committee in areas related to formulary management, medication use, and regulatory initiatives. Monitors and assesses the activity of the Clinical Staff Pharmacists, residents, and/or students and plays an active role in their ongoing development. Completes drug information requests and participates in educational efforts related to medication use. Identifies, organizes, and completes performance improvement projects related to medication use.

Job Duties

  • Ensures quality of care through formulary and/or medication management and quality improvement projects.
  • Evaluates patient medical profiles for appropriateness of medication therapy based upon patient specific medical history, age, and physical status, and makes/documents drug therapy recommendations.
  • Monitors, evaluates, and guides the provision of pharmaceutical care through staff/resident/student development and education.
  • Supports patient care functions of the network, especially in area of specialty.
  • Demonstrates a continual enhancement of clinical knowledge, professional development, and promotion of the pharmacy
    profession through training staff and/or students and residents.

Minimum Qualifications

  • Bachelor's Degree Pharmacy or
  • Doctorate Pharmacy
  • 1 year PGY-1 Residency
  • Ability to accurately dispense and/or monitor medications.
  • Ability to work effectively in a team environment.
  • Ability to make independent judgments under stressful situations at times.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  • BCPS - Board Certified Pharmacotherapy Specialist - State of Pennsylvania within 2 Years
  • RP - Registered Pharmacist - State of Pennsylvania

Preferred Qualifications

  • BCPS - Board Certified Pharmacotherapy Specialist - State of Pennsylvania Upon Hire
  • PGY-2 Residency in Critical Care (Cardiology focus preferred)

Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
