We are seeking a Case Manager working with our IDD population to join our team in Chico, Ca.
The Case Manager is a person responsible for serving as the primary advocate for the individuals and for ensuring that monitoring functions are completed appropriately. Data collection and creation of ISP. This may encompass traveling to and the responsibility for a defined number of homes in the community . The Case Manager is required to travel to any day program for the individual. The Case Manager is the liaison between the facility, individual's family, and outside agencies. Provide in- Service training to House Managers and Direct Care Professionals

The ideal candidate will possess the following qualifications:
* Bachelor of Arts degree in a social services or human services discipline and a minimum of two (2) years' experience in a leadership or management capacity as required by State and Federal regulations, and the approval from D.D.S. in writing that the requirements for the position are met and approved.
* Must have completed, or be willing to complete the QIDP certification course within six (6) months of hire.
* Writes clearly and communicates well orally. Supports ideas, conclusions and recommendations with logical and understandable presentation of analysis and research. Self-motivated and able to work independently. Basic Microsoft Office computer skills.
* Strong attention to detail and organizational skills. Make independent judgments and decisions. Exercise diplomacy, tact and good judgment consistently.
* Valid Driver's License and reliable transportation
