Job Summary
Responsible for managing patients and coordinating the services provided by multiple disciplines within and outside the health system. Maintains a collaborative practice agreement with specific physician(s). Exercises independent judgment in the assessment, diagnosis and initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates QA/QI studies to measure outcomes of care.
About our Team
We are typical Family Medicine office seeing a wide variety of ages and medical concerns. With having students, we strive to provide a great educational experience by demonstrating the breadth of Family Medicine including some of our providers doing obstetrical care, some managing opioid use disorders, some doing procedures like joint injections and skin procedures. We also have had a relationship with refugee groups and have many limited English speaking patients. Our provider group does a great job covering for each other, and has a great collaborative approach; including having an onsite MSW, Pharmacist, and Dietician that rotates in our office.
We have eight Attending Physicians, one Nurse Practitioner, and nine Resident Physicians. This is where our Resident Physicians learn how to practice outpatient primary care
