The Care Coach provides comprehensive initial and follow-up assessments in client's home, in accordance with program protocols

ESSENTIAL JOB FUNCTIONS:

  • Under the direction of the Senior Director and/or Supervising Clinical Consultant, provides comprehensive initial and follow-up assessments in client's home, in accordance with program protocols.
  • Utilizing motivational interviewing, identifies client and family service needs and goals.
  • Develops a set of recommendations to meet client/caregiver needs and goals.
  • Provides emotional and behavioral support to clients/families.
  • Provides or arranges for client/family education on dealing with dementia as indicated in recommendations.
  • Assists client/family to arrange desired services/supports from other IOA programs and community resources.
  • Initiates schedule of calls/video/e-mail sessions with client/family to carry out above responsibilities.
  • Provides phone/video/e-mail support for client-initiated unplanned calls or contacts.
  • Completes electronic client records in an accurate and timely manner.
  • Actively participates in program team meetings.
  • For support group series: facilities bimonthly caregiver support group.
  • Conducts community education and training events throughout Bay Area and on webinars.
  • Contacts all prospective caregiver coaching clients within 24-28 business hours and every 2 business days thereafter to maintain strong pipeline.
  • Manages pipeline updates and reports out weekly activities in standing meetings and to the Companioa Senior Director.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS:

  • Experience working with older adults in service settings and caregivers.
  • Strong interpersonal, organizational and customer service skills with the ability to work well in a team.
  • Experience in a role requiring personal initiative and strong problem-solving skills.
  • Computer proficiency in MS word, Excel, and Outlook.
  • Bachelor's degree required, master's preferred. Preferably in social work, health sciences, public health, gerontology, sociology, counseling, or related fields.
  • At least 2 years of direct experience working with individuals living with dementia or related cognitive diseases, or specific related education or licensure in lieu of direct experience

COMPENSATION:

Range: $ 66,571.21 to $77,222.60/annual

This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factor

This range does not include any additional equity, benefits, or other non-monetary compensation which may be included.
