Assertive Community Treatment Program (ACT)

As a member of the ACT Team, the Psychiatrist is to meet all New York State Office of Mental Health (NYSOMH) standards for ACT Teams as well as all Bellevue/ H H policies and procedures.

Job duties: The psychiatrist, in conjunction with the team leader, has overall clinical responsibility for monitoring patient treatment and staff delivery of clinical services. The psychiatrist provides:

- psychiatric and medical assessment and treatment;
- clinical supervision, education, and training of the team as needed;
- development, maintenance, and supervision of medication plan and administration
- psychiatric and medical treatment plan and procedure

The following responsibilities are unique to the Psychiatrist position:

Provides clinical leadership, along with the ACT Team Leader, to the ACT team in assessment, treatment planning, general healthcare, medical and psychosocial approaches.

Collaborates with each nursing practitioner in assessment, treatment planning, general healthcare, medical and psychosocial approaches, and a review of each ACT patient's progress and treatment.

Provides consultation to other ACT team members regarding the patient's medical and psychiatric care, including pharmacologic management needs.

Provides rare after hours or weekend support to ACT staff member on call.

Works with patients and staff in developing and implementing person centered individual treatment plans which address psychiatric issues.

Conducts initial and follow up psychiatric assessments appropriate, including MSE, psychiatric history, and establishing DSM 5 diagnoses.

Observes all standards of personal safety while assessing patients in the office and in the community.

Regularly assesses patients and prescribes psychotropic medications via office AND home visits.

Provides psychoeducation to patients and their families regarding medications/symptoms/illness/side effects when appropriate.

Provides crisis assessment in the office or community as needed and collaborates with acute and long-term providers via in-person visits, telephonic follow-up, etc. as appropriate.

Collaborates with other service providers as necessary (i.e., inpatient psychiatrists, primary care physicians, etc.).

Attends ACT team meetings when on duty in order to facilitate an integrated, multidisciplinary team approach to service.

Responds to text messages and emails from ACT team members regarding patient care in a timely manner.

Maintains compliance with all applicable practice standards and guidelines

Maintains client confidentiality and adherence to HIPAA requirements at all times

Completes all required documentation in a timely manner consistent with agency guidelines

Maintains agency and program required productivity and professionalism standards

Reports to Division Director and Medical Director for supervision and evaluation.

The ACT Psychiatrist shall be appropriately licensed to treat all ACT team clients and to provide supervision as well as education to the ACT team staff
