Requisition ID req30999 Working Title Sr Program Manager Position Grade 14 Position Summary The Office for Community Health (OCH) is a key resource to New Mexico's communities and operates under the Executive Vice President of Health Sciences. OCH's many programs offer a diverse set of resources linking community health needs with University resources in all mission areas-clinical services, education, research and health policy. This position will work under the Vice President for Community Health and will oversee the large Intergovernmental Transfer program (IGT). This new program encompasses three main areas: Screening and tracking the Social Determinants of Health for UNMH patients; a call center to follow up with discharged patients; and coordinating with local resources to serve discharged patients. The Senior Program Manager will interface with local Health Extension Regional Officers (HEROs), Community Health Workers (CHWs), and Research and Evaluation, to create systems connecting UNMH patients with services addressing social determinants of health to ultimately improve patient health outcomes and reduce readmissions. IGT will involve several different initiatives including working on social determinants of health (SDoH) within the UNMH system, maintaining contact with discharged patients, and identifying local resources in communities around NM. OCH is seeking a strong, experienced strategic and effective leader with the ability to think quickly and change direction as identified by leadership. The Senior Program Manager will oversee this new program and work well with many different components of OCH and with many community-based organizations. The successful candidate will have experience managing complex projects, will be able to work across silos, oversee budgets and create reports, and will have excellent communication skills. The successful candidate should have a pragmatic approach to planning, implementation and innovative problem solving to help the staff at OCH to deliver measurable, effective results that help make the mission of the OCH and vision of the UNM Health Sciences Center a reality. Duties will include leadership and strategic planning, financial (budget management and forecasting) and program management, evaluation oversight, and ensuring programs/projects are on track to meet deliverables and timeline deadlines. Conditions of Employment Minimum Qualifications High school diploma or GED; at least 7 years of experience directly related to the duties and responsibilities specified; two of which are management level experience. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
