STATUS: FT/40hrs/wk - Flex Schedule

The Street Level Caseworker will provide Mobile Outreach, Relational Case Management and Community Navigating. The primary objective of this position is to walk alongside those experiencing homelessness to identify and overcome barriers and to connect them with resources in the community. They will work in concert with the Corps Officers to ensure that the Officers can provide pastoral care; bring outreach and engagement opportunities to the unsheltered in Tempe. They will help facilitate and take part in mobile outreach and heat relief, participate in community networking events and meetings. The Street Outreach Caseworker will develop and maintain files related to programs, properties, and general matters. Managing and tracking financial assistance funds and resource inventories, collaborating with other departments and agencies to provide services or referrals to clients.

QUALIFICATIONS:



  • Associate degree in a Human Services related discipline. Minimum Two years of direct case management or experience may be substituted for degree requirements.
  • Experience working with clients in crisis and the strong ability to effectively utilize stabilization techniques that include trauma-informed care, motivational interviewing, and de-escalation techniques.
  • Bilingual English/Spanish preferred
  • Experience working with homeless, at-risk veterans, and families preferred.
  • Strong knowledge of the cycles that affect at-risk individuals/families and those experiencing homelessness.
  • Knowledge of community resources, particularly housing for low-income individuals, available in the service area.
  • Assist with social service client intake in the field
  • Excellent knowledge of and ability to use current Microsoft software computer programs, including Word, Excel, PowerPoint and Publisher, as well as e-mail, Adobe Acrobat and the internet
  • Excellent interpersonal, telephone and communication skills, both written and verbal
  • Ability to communicate and work effectively with a diverse group of consumers, staff, and community partners.
  • Ability to be professional and courteous in keeping with Christian standards of leadership.
  • Excellent detail, organization, and file management skills
  • Demonstrated dependability, maturity, and initiative; must be able to maintain program/guest/agency confidentiality and treat everyone with dignity and respect. Ability to work alone and as a part of a team.
  • Ability to maintain a self-motivated and self-directed schedule, address multiple projects in a timely manner and take projects from beginning through completion
  • Ability to work with people of diverse backgrounds and circumstances
  • Knowledge of the Homeless Management Information System (HMIS)/Wellsky.
  • Must possess a valid Arizona driver's license and pass a motor vehicle record check.
  • Must successfully pass Defensive Driving courses provided by The Salvation Army
  • Reliable transportation.
  • Must be able to successfully complete a criminal background check.
  • Must be at least 21 years of age.


RESPONSIBILITIES:



  • Develop and maintain a schedule to reach and engage individuals experiencing homelessness in the Tempe Community.
  • Update and maintain a service and referral information list of community partners within the Tempe and surrounding areas working to build and expand relationships with these agencies and to provide warm handoffs with referrals.
  • Conduct initial assessment to determine client's specific needs.
  • Conduct Coordinated Entry Interviews entering information into HMIS/Wellsky.
  • Maintain weekly contact with clients building relationships and being present in the event they are ready to engage towards stable housing, employment, education, or other economic and social self-support; and while they are on the path towards sustainability.
  • Organize, inventory, maintain, and order stock of essential supplies.
  • Facilitate, organize, and train and heat relief volunteers and staff.
  • Work as a bridge between mobile programs and Corps programs.
  • Work with the Corps Officers to refer individuals who request pastoral care.
  • Provide reports on success of program.
  • Perform other administrative and operational functions as requested.
  • Assist in intake, screening, and implementation of material assistant opportunities while tracking funding availability and inputting data into Wellsky.
  • Track, analyze, and input data relevant for county, donor, and internal stakeholders.
  • Coordinates services with other agencies such as mental health, employment, housing, substance abuse programs, and vocational services.
  • Must be willing to travel throughout the city of Tempe and work in remote locations at times in extreme heat.
  • Engage with community partners to maximize community partnerships and resources.
  • Perform additional duties as directed by Corps Officer as related to the position of the Street Level Program.


PHYSICAL REQUIREMENTS:

While performing the duties of this job, the employee is regularly required to walk, stand, climb, balance, stoop, kneel, crouch, crawl and reach with hands and arms on a continuous basis. Ability to work in extremely hot summer weather and moderately cold temps in the winter. The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 75 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to focus. The employee must have the ability to operate a telephone, desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
