St. Ann's Community is currently looking for compassionate Social Work Manager for the Portland campus. The primary purpose of your job is to manage the day-to-day functions of the Social Work Department in St. Ann's Home, Wegman Care Center, and Leo Center for Caring.

Job Description:



  • Coordinates the social work program for St. Ann's Home, Wegman Care Center and Leo Center for Caring


  • Plans, develops and administers all Social Work programs and services
  • Demonstrates knowledge of general social work practices
  • Maintains a quality improvement program that includes policy review, auditing etc.
  • Ensures that problems emergent in Social Work practice are identified and resolved in a timely fashion
  • Ensures that Social Work practice conforms to regulatory and professional standards, Social Work Code of Ethics and to St. Ann's Mission
  • Establishes and maintains an on-going relationship with residents/patients and significant others. Assists residents/patients through this relationship to adjust to any medical, social, emotional problems which may arise
  • Continually assesses the resident's/patient's psychosocial strengths and needs and providing counseling, support groups, emotional support and referrals to meet identified needs
  • Acts as a liaison between the family and St. Ann's staff, providing supportive services and interpreting policies and procedures
  • Provides social work input to address psychosocial aspects of all resident/patient care needs such as behavioral issues, psychotropic medication use and physical/cognitive decline
  • Provides resident/patient and significant others with hospice and grief counseling and support
  • Coordinates Social Work activities with those of other major departments to include; Administration, Admissions, Nursing, Medical, Dietary, Recreation Therapy, Facility Services, Finance & Pastoral Care
  • Actively maintains quality working relationships with other departments
  • Maintains liaison relationships with the community agencies essential in the execution of the Social Work Mission in St. Ann's (i.e.: Department of Social Services, Home Health Agencies, Residential Health Facilities, local hospitals and referral sources)
  • Serves as liaison to the Hospice Agencies currently contracted with St. Ann's Community
  • Serves as liaison between United Health Care Optum personnel and social work department
  • Serves as liaison to the Sr. Psychology Services Program
  • In conjunction with Life Enrichment Coordinator, actively pursues ways for St. Ann's Home to move forward in culture change journey
  • Actively seek out and support education regarding person centered care for self, staff and others within the organization
  • Serve as a role model and assists the organization in ensuring that the needs, routines, wants and desires of the individual are supported and met
  • Demonstrates knowledge and adherence to state and federal regulations in administering the St. Ann's, WTCC and LCC social work policies and procedures
  • Reviews, updates, and/or develops new social work policies and procedures annually
  • Ensures that policies and procedures are communicated to staff effectively
  • Conducts bi- weekly staff meetings and maintain minutes of meetings
  • Completes monthly rounding with all direct reports and submits to supervisor
  • Assures quality working and liaison relationships with internal and external parties
  • Creates and maintains a positive working environment
  • Coordinates St. Ann's Home Resident Council, and Family Council
  • Coordinates St. Ann's Home Quarterly Newsletter with Life Enrichment Coordinator
  • Maintains timely documentation of events and all required paperwork including but not limited to MDS, CAAS, ICCP
  • Coordinates resident/patient group meetings including: Resident Council, and family/resident/patient support and informational groups
  • Coordinates the transportation needs for residents/patients on an as needed basis
  • Assists resident/family and significant others in making application to MCDSS for Medicaid and provides interpretation and assistance with health related forms
  • Provides supervision of social work students from area colleges
  • Facilitates the application of Resident's/Patient's Rights in daily care issues.
  • Provide rotating weekend social work coverage
  • Performs tasks as assigned by the Manager.



Candidate Requirement:



  • Master's Degree in social work (M.S.W.) or gerontology preferred, Bachelor's degree in social work (B.S.W), gerontology; four to ten years related experience and/or training; or equivalent combination of education and experience.
  • Thee year min. experience in Geriatric Social Work, including supervisory experience.
