Clinical Social Worker

In collaboration with patient/family, social workers, physicians and interdisciplinary team, we are seeking aClinical Social Workerwho will provide direct psychological, social and practical care including social work assessments, problem identification, goal setting and implementation of care plans. Collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to, developing action plans to mitigate psycho-social barriers to discharge, patient/family engagement and education, and employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.

This position provides clinical social work services in the Post-Acute COVID-19 Team (JH PACT) outpatient clinic at Johns Hopkins Bayview Medical Center. This clinic is a collaboration between the Division of Pulmonary & Critical Care Medicine and the Department of Physical Medicine and Rehabilitation. The goal is to address the unique needs of COVID-19 survivors as an inter-disciplinary team. The Clinical Social Worker is responsible for psychosocial assessment and clinical interventions including behavioral health needs, coping and adjustment to illness and hospitalization, crisis intervention, non-pharmacological symptom management, family meetings, and community resource referrals, etc. Includes collaboration with rehabilitation psychologist, psychiatrist, and community health worker. May include facilitation of a peer support group.

Specific Duties & Responsibilities

  • Serves as a clinical resource expert in a specialty area; provides clinical oversight and consultation and functions as a mentor for others.
  • May engage in academic work including research, grants, publications and education.
  • Conducts face to face screening on all patients in assignment area with positive indicators for social work consultation.
  • Completes full psychosocial assessment and plan of care for patients and families who require consultation.
  • Provides direct psychosocial care and psychosocial transition planning for a full case load of patients in assigned area.
  • Works in a collaborative practice model, facilitating care coordination with the multidisciplinary treatment team for care progression to optimize patient outcomes.
  • Adheres to departmental and regulatory documentation standards and formulates clear descriptions of the problems, goals and interventions.
  • Adheres to timely completion of medical record documentation.
  • Maintains a current and productive network of referral resources.
  • Seeks out local, regional and national community resources to meet patient need.
  • Assists with resource referral, capacity determination, legal guardianships, adoptions, psychiatric referral, and assessment of abuse/neglect/IPV, end of life and charity care.
  • Identifies need for and facilitates family meetings.
  • Leads or participates in interdisciplinary rounds if applicable for assignment area and articulates social work role to the team members.
  • Participates in quality improvement activities; may serve on committees within the medical center and community.
  • May make home visits as required by assignment.
  • Provides consultation and direction to staff (students, support staff).
  • Serves as a mentor to other social workers, particularly those at the entry level.
  • May provide clinical social work supervision per COMAR regulations if approved as a clinical supervisor by the Maryland Board of Social Work Examiners.
  • Will be required to provide coverage during absences, vacancies and on call.
  • Other duties as assigned.


Minimum Qualifications
  • Master's Degree in Social Work.
  • Licensed as LCSW or LMSW in the State of Maryland.
  • Two year's experience in the field of clinical social work.


Preferred Qualifications
  • Knowledge of cognitive behavioral therapy, interpersonal therapy, or dialectical behavioral techniques.
  • Experience in moderating (peer support) groups.
  • Experience working in a medical setting.
  • Experience with college and/or graduate and professional students and young adult population.
  • Expertise in advanced application of social work knowledge of psychodynamics with a broad application of SW theory.
  • Knowledge of patient population needs and understanding of program elements required to mitigate deficits.
  • Supervisory experience within a health care environment (team, functional unit, hospital).

Starting Salary Range: $48,000 - $84,100 Annually ($66,000 targeted; Commensurate with experience)
