Description

The Nurse Case Manager supporting Children's School Services will:



  • Provide case management services to the School Health Services Program students and their families or caregivers.
  • Assist with assessment, participate in planning and implementation of intervention and ensure follow-up and coordination of services.
  • Engage in outreach activities in the community.
  • Assist with educational tasks related to the SHSP.
  • Support students in 504 and IEP meetings.
  • Conduct medical needs assessments of new schools in the program.
  • Support care coordination of children in during school.
  • Collaborate with Professional Development team during orientation and annual skills sessions.
  • Schedule telehealth visits with specialty clinics in CN enterprise and other community providers.

Qualifications

Minimum Education
Bachelor's Degree in Nursing (BSN Required)

Licensed Registered Nurse (RN) in the District of Columbia (required)

Minimum Work Experience
2 years Experience interacting with client families in areas of service (i.e., Medicaid application, financial counseling, specialty care coordination, etc.) (Required)

2 years of Pediatric or School Health experience (preferred)

Functional Accountabilities
Patient Services



  • Assist with intake and needs assessment of CPHC patients/families.


  • Participate in patient plan of care development, reevaluate and revisit care plans.


  • Implement, coordinate care plans through provision of support, consultation, liaison and crisis intervention services.


  • Identify appropriate resources, make referrals, and follow-up on referrals in collaboration with other hospital staff and community organizations. Monitor the progress of care plans by reviewing patients' access of resources.


  • Encourage adherence to plan through telephone contact and supportive interventions and education.


  • Anticipate transportation needs for clinic appointments and help patients arrange transportation.


Teamwork



  • Actively participate in clinic/department meetings and in department/hospital committees and task forces.


  • Participate in in-service and professional meetings.


  • Supervise as necessary.


Documentation



  • Document case management activities in the medical record in accordance with departmental standards and JCAHO and hospital policy.


  • Complete required statistics and administrative reports for the department.
