Social Worker PT Days

Job Description:

8:00am-4:30pm on Thurs, Fri, and Saturday

Part Time 24 hours weekly

The Social Worker (BSW) is a dedicated position responsible for discharge planning and case management of patients and their families within the hospital setting. This involves hands on interaction with aiding patients and/or family members in achieving an effective transition from hospital to post-hospitalization care through assessment, planning, relaying information, and referral to appropriate community agencies throughout the discharge planning process.

Job Duties:

• Complete a home needs and social determinants of health assessment to identify any medical support, services, equipment, or community resources needed post-hospitalization.
• Provide individual and family counseling to patients facing medical crises, chronic illnesses, end-of-life issues, or other challenges, utilizing supportive and therapeutic techniques.
• Collaborate with healthcare providers, including physicians, nurses, therapists, and case managers, to develop and implement integrated care plans that address patients' social and emotional needs.
• Coordinate referrals to community agencies, social services, and other resources to facilitate continuity of care and address patients' social determinants of health.
• Recognize indicators of abuse, neglect and exploitation of adults, children and elders, ensures appropriate reporting to authorities, and works in cooperation with protective service agencies to address medical and emotional needs of victims.
• Provide discharge planning services to patients as part of the multidisciplinary team, including third party payers and other healthcare providers to provide patients with a holistic plan of care, facilitate movement of the patient through the system, and arrange an appropriate discharge plan

Requirements:

• Bachelor of Social Work degree from a school of social work accredited by the Council on Social Work Education required.
• Must obtain Ky license as a Licensed Social Worker preferred upon hire within 2 years

Preferred requirement:

• Two years of social work experience preferred. Case Management Certification (CCM) preferred
