Job Description: The Homecare RN Case Manager serves as a key member of the interdisciplinary team and actively manages and directs resource utilization to achieve the highest quality outcomes during a patient care experience. The incumbent oversees the effective coordination of services and manages issues in the following main areas: admission and discharge, team conference and interdisciplinary plan of care communication, patient and family education/relations, payor relations, and coordination of care for all disciplines and total fiscal management. The incumbent performs ongoing utilization and quality reviews and acts as a liaison between the payor and patient while assuring that cost-effective treatment is provided by the team. The incumbent performs RN Shared Leadership duties by supporting the fundamentals of safety, quality, experience, and stewardship by providing clinical expertise and leadership to their department. The Homecare RN Case Manager has significant leadership responsibilities (e.g., leading change, scheduling, maintenance of staff competencies, new employee selection/orientation, constructive discipline, shared leadership council representation, caregiver check-ins). The incumbent has accountability for coordination of a cohort of caregivers and actively participates in the overall operations of the department, development of department practice, and provision of patient care with specified accountabilities for implementation and outcomes.What does it mean to be a part of our Homecare & Hospice team?
