Mary Free Bed Rehabilitation

The Advanced Practitioner Provider will be joining a team of Physical Medicine and Rehabilitation providers in acute and post-acute care locations within the Mary Free Bed system. Clinical responsibilities include the independent assessment and management of illnesses, injuries, and other conditions that result in functional impairment. The APP will work collaboratively with their clinical team and supervising provider(s), along with interdisciplinary teams in the acute and post-acute setting, to help direct patients along an optimal clinical and rehabilitative continuum of care. The APP will actively engage with patients, as well as their family/caregivers, to provide necessary clinical education, support, and forecasting of patients' functional recovery and ongoing expectations. The APP will help guide patients throughout their acute and post-acute clinical course while contributing to the management of their acute medical needs, optimizing efficient throughput and resource utilization along the continuum of care, and positioning patients to maximize their functional recovery.

Essential Job Responsibilities



  • Clinical Required: Provide patient care, treatment, and services within the scope of their license, certification or registration and as required by Federal and State laws and regulations.
  • Functions in the role of Advanced Practitioner Provider to provide coordinated comprehensive care delivery in the inpatient setting. Writes orders/prescriptions for standard formulary medications, laboratory studies, radiographic studies and consultations, coordinating the scheduling of new patients, diagnostic evaluations, follow-up exams, laboratory tests, referrals to home care, social services, and other community agencies, etc., in order to ensure continuity of care and coordination of services for patients.
  • Provide services in the inpatient setting which may include, but are not limited to:

    • Completing patient follow up visits, and occasionally new consult encounters, in the acute care and long term acute care hospital settings. This will be completed at the direction of the APP's supervising physician, though also with the APP's own initiative to see patients requiring follow up.
    • Providing comprehensive Physiatric medical care to patients encountered in the inpatient setting. These clinical areas of expertise may include: pain management, wound care, orthotic/prosthetic evaluations or recommendations, spasticity management, brain injury management, spinal cord injury management, musculoskeletal related complaints - as well as other conditions common to PM&R practice settings.
    • Collaborating with the supervising physician and Mary Free Bed clinical liaisons, as well as acute care interdisciplinary teams, on a daily basis. These communications will assist in determining which patients require follow up, and will contribute to safe, efficient, and appropriate timing and decision making for patients transitioning from acute to post acute levels of care.
    • The APP will assist in development and application of a virtual consult service. This will be designed to serve patients in rural and outlying communities, and to expand patients' access to appropriate post-acute levels of rehabilitative care.
    • Within the acute care setting, maintainance and growth of the PM&R consult and follow up service depends upon excellent communication and positive working relationships with acute care teams. The APP will be asked to intermittently round with acute care teams on a rotating basis. This may include, but is not limited to: Neuro/Medical ICU, Trauma Surgery, Neurology, Neurosurgery, LTACH medical and specialty teams.
    • May be asked to participate in acute floor/LTACH team conferences to review the medical and functional progress of patients within a post-acute setting.


  • Documents in the medical record and dictates information as appropriate. Demonstrates competence in navigating and appropriate documentation in electronic medical record.
  • Assesses the educational needs of the patients, significant others, health care team, and the community. Develops, implements, evaluates and revises teaching strategies and educational programs specific to the patient's medical and psychosocial needs.
  • Measures the outcomes of care and evaluates services across the continuum of outpatient services. Makes recommendations to improve the quality of patient care provided and utilizes the scientific method to research clinical problems.
  • Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and attending related conferences, seminars and other related activities. Maintains the professional competence, knowledge, and skills necessary for all responsibilities. Completes required training and education, including department specific requirements.
  • Ensures that services are provided in accordance with state and federal regulations, organizational policy, and accreditation/compliance requirements.
  • Performs such individual assignments as management may direct. Establishes and maintains effective working relationships within the organization. Provides information to senior levels of management on the implication of policies and procedures being formulated and recommends specific action.
  • Acts as a patient advocate, crossing multidisciplinary lines to meet patient needs. Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Mary Free Bed Rehabilitation Hospital. Acts as a consult in area of expertise.
  • Serves as a resource for patients, families, team members, and community for information related to patient populations served.

Customer Service Responsibilities

Essential Job Qualifications



  • Certified and licensed as a Nurse Practitioner (NP) by the State of Michigan.
  • Master's Degree in Nursing
  • Requirement Prior experience in a rehabilitation related role or practice setting
  • Current Basic Life Support (BLS) certification



Preferred Job Qualifications



  • Previous experience as a NP specifically within physical medicine and rehabilitation
  • Previous experience functioning in a collaborative role
