Job Description: The Clinical Therapist utilizes evidence-based treatment to care for patients. They will use their clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.Mental Health Integration (MHI) is collaborative mental health care that is integrated into everyday primary care practice. PCPs and office staff collaborate with care managers and mental health specialists to implement individualized strategies for patients and families. As the Mental Health Specialist within the MHI model, you will work directly with the Primary Care Provider and team to clarify complex patient diagnoses, plan effective treatment, and provide solution-focused interventions. This improves clinical decisions, helps patients and families receive services within primary care, and reduces PCP burden. This position provides care appropriate to the established plan of care in the assigned patient population. Job Essentials:
