We are seeking Case Management Director of Operations to join our team in our Broussard, LA location.
Earn: $95,000/year + $7,500 yearly mileage stipend
This position will be responsible for overseeing and optimizing the operational aspects of the Nursing Home Transition and Diversion Case Management Program. The patient population includes members identified as part of the DOJ Target population, including individuals with mental health diagnoses, complex medical conditions, and/or social-economic and co-morbidities. This role is pivotal in ensuring that case management processes are efficient, effective, and aligned with the organization's strategic goals. The director will lead a team of professionals, manage resources, and collaborate with various departments to drive operational excellence and improve client outcomes.
General Responsibilities:
* Ability to work with other departments within the agency and with outside stakeholders to meet individual needs.
* Attend all required meetings inside and outside the agency.
* Collection and interpretation of the statistical information to monitor program trends and performance.
* Ensure adherence and compliance to program best practices, Merakey Policies, and outside agencies.
* Ensure compliance to state, local, federal and contract requirements as well as requirements from payors.
* Facilitate ongoing assessment, evaluation and planning of programs
* Oversee quality and quantity of services provided by program
* Participate in CQI efforts
* Prepare for site visits and audits
* Provide administrative on-call availability
* Provide clinical leadership in development and delivery of modalities, most effective treatment standards, policies and procedures
* Provide direct supervision of staff to ensure productivity
* Provide ongoing administrative program direction in development, program evaluation, and program planning
* Resolve conflicts and problems within program and outside agencies
* Responsible for accurate completion of program activities
* Provide on site/in city presence during disasters, managing operations
* On call duties as assigned
* Follow all Merakey policies including but not limited to standards of conduct, confidentiality requirements, standards of professional behavior
* Document in accordance with Merakey policies
* other duties as assigned.

The ideal candidate will possess the following qualifications:
* Masters degree in human services/related field.
* Minium Experience preferred: 5 years of clinical experience working with adults with severe mental illness + 3 years of experience in a supervisory role.
* Valid LA Driver's License and appropriate state and federal clearances.
* LPC or LCSW with a current unrestricted license to practice in the state of Louisiana.
