Location Information Saint Threse is a faith-based, nonprofit organization that offers a continuum of care and services available at one of our four Twin Cities communities: Brooklyn Park, Shoreview, Woodbury, and our newest site opening in Corcoran in September 2024. Since 2016, Saint Therese of Woodbury is our youngest community. Social vibrancy, distinctive living spaces and a continuum of care blend to create a unique senior living experience. We offer 216 independent, assisted living and memory care apartments; 56 skilled and transitional care suites. Responsibilities The RAI Coordinator is responsible for accurately completing the RAI process/Resident Interviews for assigned residents, attending resident care conferences, reviewing charts and admission information and reporting areas of concern to the appropriate Clinical Coordinator, and attending discharge meetings when directed. Other responsibilities will be:
Education: Licensed as a RN or LPN in Minnesota, license in good standing. Certification in Gerontology preferred. Experience: Minimum three years long-term care experience.
