Title: Care Manager - Care Management Location: Maury Regional Medical Center - Columbia, TN Position Shift: PRN Position Summary: The Care Manager coordinates care with the patient, family, and healthcare providers to provide a seamless transition from the hospital to the community. Manages all aspects of transition/discharge planning. Promotes the achievement of clinical and resource outcomes throughout the continuum of care. Responsible for facilitating appropriate length of stay and readmission prevention/reduction activities. Educates and manages chronic disease patients across the continuum of care. Acts as the key information and education source to the interdisciplinary team, including physicians. Analyzes current systems and variances to identify improvement opportunities. Works to promote quality of care through collaboration with all team members, patients, and families.
