Job Title: Case Manager - DEN New Heights

Department: Center for Opportunity Rehabilitation and Employment

Reports To: Program Manager (DEN)

Location: 1350 W. 13th Ave, Denver, CO 80204, subject to change

Hours: Monday - Friday 8:00am - 4:30pm, occasional weekends and evenings.

FLSA Status: Non-Exempt

Salary: $22.00 - $29.32 per hour

Schedule: 40 hours per week

MISSION

For over 50 years Bayaud Enterprises has been an innovator in the community in reducing hurdles for individuals to engage in the workforce and attain self -reliance. We do this through personalized Employment Services, Social Enterprises (Business Solutions) which provide employment for those needing supported employment, and Individual and Community Services like providing free laundry with the Laundry Truck and support in finding housing and accessing different resource programs through the help of our Navigators. We create Hope, help to find Opportunities, and inspire Choices.

DEPARTMENT

Bayaud Enterprises is partnering with Denver Department of Housing Stability (HOST), Colorado Department of Correction (CDOC), Denver Sheriff's Department (DSD) and Denver International Airport (DEN) for their job readiness program, DEN New Heights. This program provides job readiness skills and paid work experience intended to find long-term employment for individuals recently released from incarceration.

SUMMARY

This position will provide diverse employment services to individuals experiencing disability, poverty, and/or homelessness. Services for participants include vocational assessments, facilitating pre-vocational and pre-employment individual and group services, supporting participants to identify job goals, execute effective job searches, obtain suitable and appropriate employment, and retain employment over time. This position will also facilitate career advancement plans for each participant.

RESPONSIBILITIES include the following:



  • Serves as part of the CORE team within Bayaud Enterprises, cooperatively working with participants to provide an array of services which may best serve the person's needs.
  • Develops a good rapport with people experiencing poverty and/or homelessness and understand their hurdles caused by or exacerbated by these conditions.
  • Connects participants to relevant health, food/nutrition and shelter resources using trauma informed practices.
  • Assists participants with money managing/budgeting.
  • Assists participants with daily crises that impede their ability to work.
  • Provides support and advocacy services to participants for assistance in maintaining access to public benefits and/or community resources.
  • Assists participants in navigating any barriers that arise, a missed court date, an active warrant, loss of personal belongings, etc. and supports them through the process of addressing these hurdles.
  • Acts as the team's intake provider, opening file in database, conducting initial assessments, and setting out the case management plan. The Case Manager keeps the file updated as milestones are met, or as new needs arise.
  • Completes all documentation and data entry as required in a timely manner.
  • Works collaboratively with a wide variety of constituents and adjusts to multiple demands and shifting priorities.
  • Works with employment team members in supporting the goals and objectives outlined in everyone's IPE (individual plan for employment)
  • Educates/informs individuals about how income may affect other human service benefits they may receive.
  • Attends applicable staff and community meetings.
  • Performs other duties to include jobsite coverage as assigned by management.


SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities


Qualifications

QUALIFICATIONS



  • Excellent Microsoft Office skills, especially Excel, Outlook, and Word
  • Ability to work in multiple databases including E-Logic and HMIS (Homeless Management Information System), and proficient in data entry
  • Excellent communication skills (written and verbal), grammar, and reasoning skills
  • Understanding of the socioeconomic conditions that impact conditions for people experiencing homelessness, disability, and severe poverty.
  • Ability to handle confidential and sensitive information without breach
  • Personal commitment to working within an integrated services model, with emphasis on employment as a key component to ending poverty and homelessness.
  • Ability to cultivate strong working relationships with community resources.
  • Knowledge of applying for SNAP (Supplemental Nutrition Assistance Program), Medicaid, LIVE and other benefits for people living in poverty.
  • Knowledge of community resources available to participants such as food banks, healthcare options, linkage to housing, etc.
  • Ability to connect participants to any other services they may need in the community.


EDUCATION and/or EXPERIENCE



  • High school diploma or general education degree (GED) and 2 years' experience in case management.
  • The candidate for this position should have considerable experience in crisis intervention, trauma informed care practices, supporting individuals experiencing poverty, homelessness and disabilities as they strive to obtain and retain employment, money management, benefits assistance issues, an understanding of mental illness, and how these influence employment situations.


LANGUAGE SKILLS

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to author routine reports and correspondence. Ability to speak effectively before groups of customers or employees of an organization.

MATHEMATICAL SKILLS

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

CERTIFICATES, LICENSES, REGISTRATIONS

Must have a valid driver's license with the ability to be insured by Bayaud's automotive carrier.

WORK ENVIRONMENT

The work environment will be a combination of being in a temperature-controlled office environment and out in the field in Colorado weather. While performing the duties of this job, the employee is regularly required to

sit and use hands. The employee frequently is required to talk or hear; the employee is occasionally required to stand, walk, reach with hands and arms, stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities by this job include close vision, distance vision, and ability to adjust focus.

ALTERNATIVE WORK PLAN

This position is not eligible for an "Alternative Work Plan." All duties must be performed in the office or in the community with participants. Special projects will be considered on a case-by-case basis.

TRAVEL

Travel will include Meetings in the community. Pick up/delivery of supplies or items needed for participants. Other travel as needed. Will drive a company vehicle for these purposes. If a personal vehicle is used for work travel, this driver will be reimbursed at the federal rate for mileage.
