Elwyn is currently seeking a Director of Admissions to evaluate all referrals and coordinate and facilitate the intake and admissions processes for Elwyn New Jersey's Adult Intellectual and Developmental Disability (IDD) Services programs including Adult Day Services, Supported Employment Services, and residential services provided through the Community Living Program. The Director of Admissions also supports any assigned projects or tasks as requested by the Executive Director of New Jersey, the Executive Director of Adult Day and Supported Employment Programs, or other Executive or designee, to ensure and promote customer satisfaction and the interests of Elwyn New Jersey.

DUTIES AND RESPONSIBILITIES:



  • Track all referrals, vacancies, and transfers of residents in Coordinated Care Platform (CCP)
  • Maintain and/or exceed budgeted census levels in IDD Programs
  • Work collaboratively with New Jersey State Department of Human Services' Division of Developmental Disabilities and third-party case management entities and other funding source representatives to procure and assess possible referrals
  • Seek applicants from referral sources and establish strong working relationships with referral sources
  • Screen applicants for potential admission of individual to receive services into residential, day and/or work services programs through tasks including but not limited to:


    • Screen referrals by phone or correspondence according to admission criteria
    • Collect and complete required assessments and evaluations based on regulations
    • Schedule all pre-application interviews
    • Select the program that would best serve the applicant, if applicable
    • Coordinate the full interview process with internal and external personnel including but not limited to the appropriate management team members, nursing, day program, supports coordination, candidate's representatives, referring agencies, etc.
    • Customize the placement process to fit the needs of the applicant and the applicant's family and/or other representatives
    • Collect, review and assess all information required by Elwyn New Jersey policy and state and federal regulations
    • Request, receive, assess and document the input of the team regarding the candidate
    • Determine applicant acceptance
    • Complete, explain, review and collect consents, authorizations, and any other information and documentation required for admission
    • Negotiate additional supports if immediately identified


  • Establish initial authorizations, annual service plans and provide social service supports including:


    • Participate with the team in developing initial plan for the resident
    • Complete and submit representative payee forms and/or other Medical Assistance documentation


  • Coordinate all involved parties for the intake and admissions systems per the admission protocols for New Jersey residential homes, and for Day/Work programs including but not limited to the process from referral to acceptance, designing forms, writing/updating policies and procedures regarding intake, writing/updating the intake piece of the program participant handbooks, service planning, case activation with funders and initiating billing within Elwyn New Jersey systems
  • In coordination with the Individual Funds department, establishes benefits for the individual's residential services with Social Security, family, and funder by working with the family, social security administration representatives and county representatives to collect required documentation and address issues to ensure that the individual is eligible for the funding required for their residential placement
  • Works with the Service Eligibility Coordinator, the Program Finance Coordinator, and Director of Adult IDD Shared Services to ensure that benefits (i.e. medical assistance) are started for the residential placement and authorization for funding is secured
  • Schedule pre-placement meeting with operational, behavioral, and medical teams; the individual seeking services and their family/guardian; support coordination representative; and any other external provider of services to review all needed services, finalize the individual support plan, review all required admissions paperwork, and establish an admissions date
  • Develop a plan to support the resident's family through transition to placement into residential, day, and/or work services program(s)
  • Assist in solving all initial logistical issues, including, but not limited to, transportation, clothing, and special medical needs (i.e. dietary restrictions)
  • Provide the initial family evaluation regarding Elwyn New Jersey's policies and procedures as they relate to individual's finances, individual rights, health and safety, and regulatory requirements to ensure transparency of all processes and procedures
  • Oversee transfers of residents and adjust vacancies as feasible to fit the needs of residential candidates
  • Provide and coordinate technical/professional support to the operational, behavioral and/or medical teams on a consultative basis, and train new Program Specialists (PSs), Operations Managers, and Directors of Operations in the admission process
  • Provide information and tours to agencies, families and individuals interested in placement planning, forwarding descriptive literature and information as necessary
  • Represent the organization at professional conferences, seminars and other public events, and work with other service organizations engaged in placement activities to generate interest, share information and describe opportunities available in Elwyn New Jersey programs
  • Participate with the full management team in new development of services and special projects
  • Coordinate projects involving interagency relationships and establish annual and departmental admissions objectives
  • Perform other duties as assigned



QUALIFICATIONS, EDUCATION AND EXPERIENCE:



  • High school diploma or graduate equivalency diploma (GED); associate degree or bachelor's degree in healthcare, psychology, social work, education, or related human services field preferred
  • Seven (7) years' experience in human services; experience working with individuals with developmental disabilities
  • Two (2) years experience in quality assurance, utilization review, billing, or related area preferred
  • Non-profit human service experience highly preferred
  • Working knowledge of funding source operations preferred
  • Knowledge of health insurances, Medical Assistance, Social Security Administration, and managed care organizations (MCOs) rules a plus
  • Demonstrated strong understanding of admissions planning and coordination services
  • Knowledge of State of New Jersey, Department of Human Services/Division of Developmental Disabilities community and residential services regulations
  • Knowledge of applicable federal and State of New Jersey, Department of Human Services, Division of Developmental Disabilities governing bodies and regulations, including but not limited to those regulating and licensing Adult Day, Supported Employments, and Community Living Programs, such as Community Care Policy and Procedures and the New Jersey Developmental Disabilities Day Habilitation Services Compliance preferred
  • Knowledge and experience in Community-based residential and adult day and supported employment program settings preferred
  • Working knowledge of International Classification of Diseases (ICD)-10 and Current Procedural Terminology (CPT) codes
  • Previous experience in state and government contracts preferred
  • Experience using/knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
  • Must have current, valid driver's license in state of residence, 3 years driving experience in the United States, and acceptable driving record
