Job Description:
- As an accredited health information management technician, prepares and codes all clinical records including medical and psychiatric diagnoses.
- As an audit specialist interprets and analyzes records to determine the code of all psychiatric and medical conditions recorded in the patient's clinical records using diagnostic classification standards.
- Utilizes the daily discharge list to verify all discharged patient charts are coded and entered into the HOST ON DEMAND system.
- Analyzes health information for research projects and special reports.
- Prepares tables and charts displaying findings.
- Interprets and evaluates technical and complex clinical records and conducts clinical chart audits.
- Audits clinical records for the presence, completeness, timeliness, and authentication of key assessments and other documents.
- Checks records to verify completeness of forms, appropriate signatures, etc.
- Defines audit criteria and documents actual patient care based on that criteria.
- Identifies patient records that do not conform to criteria and makes recommendations for corrective action.
- Serves on various facility-wide oversight committees concerning Health Information.
- Conducts highly focused, comprehensive, and statistical surveys of clinical records.
- Prepares reports and other information on clinical pertinence for use in professional staff peer review activities and support oversight committee activities.
- Review petitions, certificates, and related documents about civil commitment.
- Distribute documents to court officials, patients, and guardians.
- Prepares and delivers Notices of Hearing.
- Prepares court orders.
- Updates local and statewide computer systems with patient's legal statuses.
- Maintains tickler systems for court-related matters.
- Liaisons with the Clerk of the Court regarding coming hearings and filing of documents.
- Serves as facility representative when testifying in court-ordered hearings on health information records.
- Utilizes a personal computer, software, hardware, and agency mainframe applications to produce special reports, charts, forms, and graphs for summaries and displays of health information records.
- Files reports, forms, and other related data into patient's medical records.
- Checks, purges, assembles, analyzes, and files records of discharged patients.
- Assists in the charge-out of medical records to staff.
- Checks, reviews, and assembles medical records of patients for transfer to other Division of Mental Health facilities.
- Coordinates release of information activities, including review of court orders and subpoenas, approval of authorizations from outside agencies, retrieval of records, and identification of requested documents.
- Maintains a list of charts that have not been coded and entered into the system.
- Maintains and retrieves reports from the Deficiency System.
- Attends and actively participates in all mandatory training.
