We are pleased to present the following position for your consideration: Overview: Director, Community Housing Heritage is seeking a full-time Director of Community Housing to lead and manage programs focused on supporting individuals in their re-integration into the community at the least restrictive level of care. The role oversees four supervised residential and permanent supportive housing locations, and supported residential housing locations. Core Responsibilities Include:
- Provide supervision and oversight to program managers and team members.
- Attend program meetings within Heritage to foster collaborative care for persons served.
- Complete integrated assessments and treatment plans.
- Oversee the implementation of services outlined in individual treatment plans.
- Engage persons served in therapeutic interventions.
- Participate in recruitment, ensuring timely filling of vacancies, and contribute to hiring and staff development decisions.
- Ensure all clinical activities performed by the team comply with organizational standards, policies, and procedures.
- Provide on-call support with other members of the residential leadership team, demonstrating flexibility and adaptability to changing program and scheduling needs.
- Participate in program development, executing the agency's strategic plan, and communicating the program mission.
- Oversee data management, including weekly, monthly, and quarterly reports, and monitor performance measures and standards set by funders.
- Lead and participate in community-based meetings.
- Coordinate communication and service provision, continuing to build relationships with area providers for linkage within our service area.
- Ensure compliance with state, federal, local, and agency standards and procedures.
- Complete all required paperwork accurately, legibly, and within established timelines.
- Perform other duties as required within the scope of the position and the employee's experience, education, and abilities.
Knowledge, Skills and Abilities:
- Team-focused with the ability to work collaboratively across multiple programs and staff.
- Knowledge of Medicaid regulations, including Rule 132/140 and SUPR 2060/2090
- Ability to manage multiple projects of equal priority concurrently
- Demonstrate and model the behaviors expected of your teams while gaining hands on experience in each role you supervise
- Excellent time management skills
Education and Experience:
- Master's degree in social work, psychology, counseling, or a related field.
- Ability to become licensed as an LCSW or LCPC within 18 months.
- Valid driver's license, reliable transportation, and proof of current automobile insurance required.
Salary Range: $75,000-$100,000 per year (dependent education, experience and licensure) Heritage also offers the following with this position:
