A Brief Overview

Children's Hospital of Philadelphia is seeking two Nurse Navigators for their Comprehensive Vascular Anomalies Program. This application yields one opening at 32 hours/week and one at 20 hours/week.

This role will provide guidance, oversight, and coordination to the following activities:
* Represents patients and families coming to CHOP from local, regional and national, and international sources including referring physicians,
hospitals, community at-large, and other ancillary agencies by coordinating their care.
* Provides on-going family support and ensures referral to appropriate support services
* Coordinates patient care services and maintains interpersonal relationships with patients, families, physicians and referral sources for the
duration of care - from initial contact through discharge and follow-up.
* Works with leadership to assist in the development and implementation of education and outreach activities for the Program.

What you will do

  • Patient Navigator Responsibilities
  • Coordinates care with referring clinicians, patients, and families to ensure ease of access and ongoing communication, continually striving to enhance the CHOP experience, focusing on family centered care.
  • Coordinates a plan of care for referral patients and families and is the person of contact with their referring physician.
  • Assesses patient's and parent's health status, health care, psychosocial and support needs
  • Provides instruction in disease process, treatment plan, health and skill teaching Coordinates referrals and collection of appropriate medical records and diagnostic information; abstracts pertinent clinical information as necessary.
  • Maintains a relationship with the patient/family throughout the course of their care, from initial contact to the end of their hospitalization, providing the patient and referring physician with updates and pertinent medical information.
  • Supervises scheduling processes by assisting in triaging new patients for urgency; assigning new patients/consults to appropriate providers; and ensuring coordinated care for patients in need of multiple provider and diagnostic appointments.
  • Identifies potential referring physicians and develops a strategy to encourage them to become CHOP referring physicians and works to establish ongoing relationships. Participates in appropriate meetings relevant to patient's management, i.e. case conference, etc
  • Outreach and Program Development / Resource Utilization and Collaboration
  • Develops and Implements strategies to increase utilization of the services, programs and physicians.
  • Identifies and evaluates new markets, to include statistical analysis of potential markets, estimates of growth, identification of client needs, and competitive analyses.
  • Identifies potential referring physicians and develops a strategy to encourage them to become CHOP referring physicians and works to maintain ongoing relationships.
  • Periodically develops, refines and updates outreach materials, such as survey tools, fact sheets, and patient/family/provider education.
  • Facilitates philanthropic activities that support programmatic initiatives.
  • Works with the Web and Marketing Teams to assist in the development of program web and print content, including patient education materials.
  • Collaborates with all appropriate CHOP departments to strengthen relationships and facilitate the referral process.
  • Meets routinely with strategic internal partners to ensure consistent messaging to referral audiences and alignment of key program initiatives.
  • Data Management
  • Establishes, tracks and maintains metrics and helps prepare related reports and resources.
  • Develops and maintains a physician/referral contacts database.
  • Works with collaborators to develop and maintain survey tracking tool for referrals
  • Provides referral demographics (reports) and analyses to support departmental growth objectives.
  • Establishes and maintains metrics in relation to outreach priorities and departmental initiatives.
  • Leadership
  • Acts as a resource and mentor for staff (i.e. administrative staff, nursing) regarding patient or family processes or procedure inquiries, and complex patient issues.
  • Continually evaluates communication processes with all stake holders (referring physicians, hospitals, CHOP Faculty and staff, families/patients, etc.) and recommends strategies to improve if needed.
  • Works to educate team members who communicate with families regarding access questions, cardiac procedures, potential course of hospitalization, etc. as needed opportunities arise.
  • Participates in the development of patient educational materials i.e. videos, web education, parent instructions, outreach materials etc.
  • Participate in community outreach educational programs and educational activities to promote quality patient care.
  • Provides coverage or support for leaders when needed.
  • Other duties as assigned

Education Qualifications

  • Technical Diploma Nursing Required
  • Master's Degree Preferred

Experience Qualifications

  • At least five (5) years nursing experience Required and
  • Previous experience in pediatrics Required
  • At least seven (7) years nursing experience Preferred

Skills and Abilities

  • Expert knowledge of pediatric nursing
  • Intermediate proficiency with office software (Microsoft Office) including word processing, and spreadsheet software (Word, Excel)

Licenses and Certifications

  • Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or
  • Registered Nurse (New Jersey) - New Jersey State Licensing Board - upon hire - Required
