What is the purpose of our Outreach & Case Management Program?

The goal of Catholic Charities' Outreach & Case Management Program is to help clients attain a greater degree of self-sufficiency and overall improvement in their lives. The program focuses on assisting clients to evaluate and prioritize their needs and then develop a sustainable plan of action.

What will I do as an Outreach Case Manager?

You will provide case management services for clients- advocating for them and linking them in times of crisis to services, community resources, and programs that will meet their needs.

  • Develop individualized care plans for clients
  • Conduct client inquires and in home assessments to identify clients physical, social and financial well-being
  • Help clients sign up for services such as Medicare, food stamps, family care, disability etc
  • Monitor cases by observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support
  • Maintain client's records by reviewing case notes; logging events, progress and reports.



The Case Manager works together with clients, at no cost, to identify issues, problem solve, and create a plan to address long term needs and transform lives. This role regularly collaborates with and refers to other Catholic Charities programs and staff as well as other local community partners and resources.

Salary: 38,000-41,000

Schedule: Monday through Friday, 8:30am to 5:00pm

Qualifications:

  • Bachelor Degree in Social Work, or related field strongly preferred
  • Two years of previous experience in case management, family services, or social services is ideal
  • Travel is required. Valid driver's license, reliable transportation and minimum liability insurance on vehicle are all required.
  • Valid driver's license, reliable transportation and vehicle insurance (liability coverage: $100,000/person and $300,000/accident).
  • Bilingual in English and Spanish is required
