Job Description:

The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patients family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients needs, broker services, coordinate care and transition plans, and advocate for patients. The CSW performs these duties under the direct supervision of an LCSW.
As a Behavioral Health Counselor with us, you need to know how to:
  • Provide comprehensive intake examinations and behavioral health assessments, and determine accurate DSM-5/ICD-10 diagnoses based on clinical information.
  • Provide behavioral health services to individuals, couples, families and groups, using a variety of treatment approaches and tools.
  • Make appropriate referrals for patients needing services outside the scope of the immediacy of primary care.
  • Maintain complete and timely clinical records on all clients.
  • Provide consultation to other staff members as requested and participate in staffing of cases.
  • Provide consultation and education services to individuals, agencies, institutions and groups as assigned.
  • Participate in staff training and attend staff meetings.
  • Review client files to assess dangerousness and violence evaluation of family violence referrals.
  • Make referrals to a variety of social services and medical resources as needed.
  • Participate in the screening process of new referrals to the department as needed.

Minimum Qualifications

  • Masters in Social Work (MSW) or related field, required
  • Current Licensed Clinical Social Worker or Licensed Clinical Professional Counselor license is required.
  • Minimum of two (2) years of clinical experience post licensure preferred
  • Current BLS certification endorsed by the American Heart Association or the ability to obtain within three (3) months of hire (with prior approval), required

This position requires hospital credentialing which takes 90 days - start date will align when credentials are granted.

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.- and -Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.- and -Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.- and -Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.- and -Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).- and -May be expected to stand in a stationary position for an extended period of time.
