Comagine Health assists Alaska Medicaid to be a responsible steward of healthcare resources by avoiding overuse, misuse and underuse of health services. From our office in Anchorage, we provide discharge planning support to residential treatment facilities located in Alaska and elsewhere in the U.S., as well as assessments of families' resource needs and readiness for discharge of their children and adolescents. Our focus is on patient safety and cost containment, while also leveraging our deep experience and knowledge of evidence-based protocols to deliver appropriate and optimized care.

In this role, you would:



  • Facilitate timely and appropriate discharges and care transition support services through collaboration with providers, community service agencies, and other stakeholders.
  • Conduct limited family assessments telephonically.
  • Provide assistance and support to the clinical review and other Comagine Health staff.

Required Skills

  • Proficiency with care transitions support.
  • Experience assessing community resources.

Required Experience

  • At least 2 years of clinical work experience with a behavioral health focus.
  • A master's degree in social work, counseling, or related field and current, active, unrestricted MSW, LCSW, LMHC, or LPC licensure in Alaska, or
  • A degree in nursing and current, active, unrestricted RN licensure in Alaska.
  • At least 3 years of care coordination experience preferred.
